Community Habilitation Specialist Job Description The Community Habilitation Specialist provides instructions including adaptive skills development; Hands-on assistance with actives of daily living; community inclusions and relationship training and support for independence in travel and transportation; Adult educational supports; development of social, leisure, self-advocacy, and informed-choice skills; and appropriate behavior development to help individuals access their communities. Duties & Responsibilities • Work directly with an assigned participant(s) on the Habilitative actives specified in the individualized service plan., • Assist the participant in skill development throughout implementation if residential habilitation plan., • Assist in the planning and/or review the Community Habilitation outcomes along with the family, participant, family advisor and program coordinator., • Submit progress notes and time sheets by entering information and hours worked on an agency provided tablet on the same day that services are provided., • Must be 18 years or older., • High School Diploma or GED., • Prior experience working with people with developmental disabilities, mental health challenges, or in direct support roles (Preferred)
