About the Program: The Juvenile Justice Services Program at Hope For Youth provides case management, restorative justice conferencing, and clinical services for up to 40 youth referred by the Nassau County Probation Department. These youth may include: * Persons In Need of Supervision (PINS) * Juvenile Delinquents (JD) * Juvenile Offenders (JO) * JD–Raise the Age * Adolescent Offenders Our program focuses on family-centered treatment services that have proven effective in reducing delinquency risk and avoiding out-of-home placement. Position Overview: The Case Planner plays a critical role in supporting youth and families by improving individual functioning and family dynamics. This is a field-based position requiring weekly contact with youth and their families throughout Nassau County. The Case Planner also delivers direct services such as: * Case management * Life coaching * Parent skills training * Coordination with schools, courts, and other community partners The Case Planner reports directly to the Program Supervisor.
