Job Description Position Overview: As a Mobile Wound Care Nurse Practitioner/ Physician Assistant, you will be responsible for providing expert wound care management in patients' homes and/or care facilities. You will assess, diagnose, treat, and educate patients with chronic and complex wounds, including but not limited to diabetic ulcers, pressure ulcers, surgical wounds, and other skin-related conditions. You will work collaboratively with physicians, nurses, and administrative leadership to ensure the best possible care for our patients. Key Responsibilities: • Conduct comprehensive assessments of patients with acute and chronic wounds, including diabetic ulcers, pressure ulcers, venous ulcers, and surgical wounds., • Develop, implement, and evaluate individualized wound care treatment plans based on patient needs., • Perform wound debridement, dressings, and other advanced modality wound care procedures., • Collaborate with interdisciplinary teams, including physicians, nurses, physical therapists, and dietitians, to ensure optimal patient outcomes., • Educate patients and their families on wound care, prevention, and self-management strategies., • Monitor and document patient progress, including wound healing and response to treatment., • Prescribe medications, such as antibiotics or topical treatments, as appropriate for wound management., • Stay updated on the latest evidence-based practices and wound care technologies., • Master’s degree in Nursing (MSN) or Doctorate in Nursing Practice (DNP) from an accredited program., • Graduate from an accredited Physician Assistant Program., • Active Nurse Practitioner (NP)/Physician Assistant (PA) license in Texas., • Wound Care Certification (WCC) or board certification in wound care (preferred)., • Minimum of 2 years of clinical experience in wound care or a related field., • Strong knowledge of wound healing, wound care products, and advanced wound therapies., • Excellent interpersonal, communication, and organizational skills., • Ability to work collaboratively in a multidisciplinary team environment., • Strong critical thinking and problem-solving abilities., • Competitive salary and benefits package., • Opportunities for professional growth and development, including continuing education and certifications., • A collaborative and supportive team environment., • State-of-the-art equipment.
