Job Description Title: Client Coordinator Department: Day Services Reports: Director of Day Habilitation Location: Day Services, NY FLSA Classification: Non-Exempt Salary: 21.63 per hour POSITION SUMMARY: The Client Coordinator is responsible for ensuring the overall operations and services re being delivered according to state and federal regulations, agency policy and procedure and according to the agency’s mission. The Client Coordinator serves to provide leadership, direction, and training in order to supervise the overall development of a specific program. Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, develop implementation strategies which outline services and supports for individuals so they may work towards the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. For new individuals, the Client Coordinator works with the Intake Department and completion of a required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. The Client Coordinator accepts primary responsibility for development of program goals and methodologies and for supporting the DSPs to correctly implement and document such activities. Position Overview: The Dialysis Nurse will provide specialized nursing care to patients undergoing dialysis treatment. This role involves patient assessment, education, dialysis machine setup and operation, and monitoring patients throughout treatment sessions in collaboration with the interdisciplinary care team. · Pay: up to $55/hour + Overtime Opportunities · Job Type: Per-Diem, Temporary (Agency Assignment) · Schedule: Per-Diem with every other weekend required · Setting: Skilled Nursing Facility / Long-Term Care · Assignment Length: Ongoing, stable placement Responsibilities: • Perform patient assessments before, during, and after dialysis treatment., • Monitor and document patient vital signs and response to treatment., • Set up and operate dialysis equipment safely and efficiently., • Administer medications as prescribed and per protocols., • Provide patient and family education regarding dialysis and home care., • Ensure compliance with clinical policies, procedures, and regulatory standards., • Respond promptly to emergencies and changes in patient condition., • Maintain accurate, timely documentation in electronic medical records (EMR). · Current and valid New York State RN license · Previous experience in skilled nursing, rehab, or long-term care preferred · Must be reliable, punctual, and able to work every other weekend · Strong clinical and communication skills · Eligible to work in the U.S. and pass background screenings Preferred: • At least 1 year of nursing experience in acute, chronic, or outpatient dialysis., • ACLS certification. · Competitive hourly rate: up to $55/hour + OT · Female-led agency that prioritizes nurse advocacy · Weekly direct deposit and responsive staffing support · Long-term assignments offering job security and consistency · 24/7 access to a supportive coordinator who understands your needs Apply today and experience what it's like to work with an agency that values your time, your license, and your voice.
