This staff member provides case management services and assists the mental health clinician in the development and delivery of services to mentally ill individuals; participates in group and individual counseling programs; acts as an advocate for the client and creates/maintains relationships with community groups; and completes documentation in case records. Emphasis is on the interdisciplinary team approach as a problem-solving process in providing comprehensive care to clients and their families. The Case Manager shall have a committed belief in mental health care with dignity for all, and that clients have the right to mental heath care information and participation in planning their own mental heath care Essential Functions: • Responsible for assisting the client to complete appropriate releases of information important to client compliance with individual plans of care., • Advocates for clients when there is a problem in the service delivery system., • Assists clients in identifying and correcting situations that contribute to mental health problems; performs crisis intervention counseling at a level not requiring licensure; and assist clinicians in planning the range of care needed to meet clients’ needs., • Responsible for maintaining assigned case load and client contacts as required by contract requirements and/or program protocols., • Candidates must be culturally competent and demonstrate ability to engage with patients of the multi-cultural backgrounds, nationalities, origins and diverse sexual preferences., • Visits clients regularly in their homes and in the community to assess their home situations, deliver services, and determine if other services are required., • Keeps accurate, up-to-date records on clients served in accordance with system standards., • Prepares and delivers oral presentations to the public regarding Clinica Sierra Vista’s mental health services program., • Works with other staff to develop community resources., • Serves as liaison with other community agencies and schools., • Develops and implements support and educational groups., • Completion of a Bachelor’s degree from an accredited college, or university, with a major in Psychology, Sociology, Human Services, Behavioral Science, Social Work or related field., • A clean drug screen confirmation., • Pass DMV background check. Job DescriptionDescription: Job Summary: In the End Stage Renal Disease (ESRD)program, RN Case Managers provide comprehensive, community-based care coordination for high-need, high-risk individuals, with a focus on those with chronic conditions requiring dialysis. Leveraging clinical expertise, especially in nephrology and renal care, the RN Case Manager ensures person-centered, interdisciplinary support to help members navigate the healthcare system and improve health outcomes. Working primarily in person and also remote, the RN Case Manager engages dialysis patients with complex medical and psychosocial needs—that include high hospital utilization, transition of care, medication reconciliation, and education on multiple co-morbidities with higher emphasis on dialysis. This position plays a vital role in bridging gaps across acute care, dialysis treatment, social services, and long-term support systems. Essential Duties and Responsibilities: • Coordinate care and provide clinical oversight for members receiving dialysis, ensuring smooth transitions between treatment centers, primary care, specialists, and hospital systems., • Use nursing judgment to assess, plan, implement, and evaluate patient-centered care plans, integrating dialysis-specific needs., • Educate members on managing ESRD, dialysis adherence, nutrition, medication compliance, vascular access care, and early signs of complications., • Collaborate closely with nephrologists, nurse practitioners, dialysis centers, and interdisciplinary teams to support optimal renal care., • Conduct comprehensive assessments, incorporating SMART goals and motivational interviewing techniques., • Provide in-person visits at dialysis centers to support member engagement and reduce barriers to care., • Advocate on behalf of members to ensure timely access to renal treatment, specialty services, durable medical equipment (DME), medications, transportation, and other resources., • Document all care coordination encounters and follow-up actions in the electronic health record (EHR)., • Track and manage hospitalizations, discharges, missed appointments, and urgent care visits, especially related to dialysis complications., • Enroll members in the ESRD program and maintain compliance with all regulatory and programmatic guidelines., • Provide culturally competent, trauma-informed care in alignment with ESRD goals and population health strategies. Minimum Requirements: Education: • Current, unrestricted Registered Nurse (RN) license in the state of practice California., • Minimum 2 years of recent experience as an RN in dialysis or nephrology care (required)., • At least 1 year of experience in care coordination, case management, or community health preferred., • Valid driver’s license and reliable transportation., • Proof of current auto liability insurance per state requirements., • Proficiency in electronic documentation systems and Microsoft Office applications. Preferred Skills and Competencies: • Strong knowledge of ESRD, CKD, dialysis modalities, and vascular access care., • Skilled in interdisciplinary collaboration and care team leadership., • Excellent verbal and written communication and clinical documentation skills., • Demonstrated ability to engage resistant or vulnerable populations., • Ability to work independently while managing a dynamic caseload., • Bilingual in English/Spanish. Job DescriptionDescription: Job Description: The Enhanced Care Management (ECM) Case Manager addresses the clinical and non-clinical needs of high cost and/or high-need members through systematic coordination of service and comprehensive care management that is community-based, interdisciplinary, high-touch, and person-centered. The ECM case managers work, primarily through in-person, with ECM members, adults and children, that have chronic health conditions, homelessness or at-risk, high hospital admissions, substance abuse, behavioral needs, and/or transitioning from incarceration. Using excellent communication skills, case managers will provide services and coordination to members to ensure continuity of care across health and social service programs as well as community based and long term-support service programs. This position requires strong interpersonal and organizational skills to build rapport with members and to coordinate referrals and care amongst various healthcare providers and community services. The ECM case manager also works with the member’s interdisciplinary team in supporting the member. The case manager engages member and member support systems to define priorities that are central to the member’s desired needs and goals.
