Job Description Location: [Amityville, NY] Program: [ISCM] Reports To: [Lisa Hasbrouck] Hope For Youth's Intensive & Supportive Case Management (ISCM) program is an out-patient preventative service that provides children and their families with the support and treatment needed to stabilize a youth's immediate living situation and prevent the placement of children into foster care or a higher level of care. The ISCM's intensive track (ICM), can accommodate the needs of up to 12 children and their families. The ISCM Program is a contract program through the Suffolk County Department of Social Services and services families throughout Suffolk County. The ICM track is designed to help prevent disruption, multiple placements and placements in more restrictive settings. Hope For Youth's caseworkers utilize proven screening tools such as the Casey Life Skills Assessment and Child and Adolescent Strengths and Needs Screening. ICM staff work closely with the DSS. Through interdisciplinary assessments, DSS and Child Protective Services provide the Hope For Youth ICM team with information pertaining to the families past and current functioning so that Hope For Youth's Intensive Case Managers can provide recommendations for going forward. Communication between Suffolk County and HFY is ongoing. Key Responsibilities To be a successful Intensive Case Manager, you must be ready, willing, and able to do the following: • Provide services to maintain the child in their current living environment and help achieve positive outcomes as outlined in the service contract or grant., • Assess and address safety and health concerns in the home environment and advocate for the child as needed., • Conduct twice-weekly face-to-face home/community visits and complete all required reports and documentation., • Maintain a caseload of up to four clients., • Perform any additional duties necessary to meet the needs of clients, the program, Hope For Youth, oversight agencies, and other regulatory obligations. How You'll Spend Your Time • Field work and home/community visits, • Documentation and case-related paperwork, • Staff meetings, supervision, and interdisciplinary team meetings, • Training and professional development, • Other duties as assigned Minimum Qualifications • Bachelor's degree in Social Work (BSW) or a related field, • Valid NYS Driver's License in good standing, • Reliable personal vehicle for regular home visits and transports throughout Suffolk County, • Strong writing and time management skills, • Flexibility to work evenings and occasional weekends, • Ability to respond to calls or emergencies via an agency-provided mobile phone or internet communication tools Physical Requirements • Ability to read and write reports/logs, • Prolonged walking, standing, or sitting, • Bending, stooping, kneeling, squatting, climbing stairs, and reaching overhead, • Repetitive wrist/finger movements for documentation, • Regular driving using a reliable personal vehicle This opportunity is being offered as Full time, Monday – Friday (flexible as needed) Only candidates selected for an interview will be contacted.
