Job Description JOB DEFINITION: The Care Coordinator functions as a member of an interdisciplinary team to provide care coordination to adults diagnosed with a severe mental illness, with multiple medical co-morbidities and/or co-occurring substance abuse disorders. The Care Coordinator advocates for and supports the client, engages with community agencies/health care providers and others on the client's behalf to ensure access to services, to increase wellness self-management and to reduce emergency room visits and/ or hospitalizations. The Care Coordinator will provide expert support and psychosocial and/or substance abuse interventions, and resources to aid in the consumer's care while maintaining focus on outcomes and best practices. The Care Coordinator must be available on an as-needed basis 24 hours a day, 7 days a week. EDUCATIONAL REQUIREMENTS: Bachelor's Degree or Master's Degree in one of the following fields preferred: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy. Degrees in other related areas may be considered. EXPERIENCE REQUIRED: • For H.S. THE JOB As an Emergency Credentialed Veterinary Technician, you're part of a team that thrives on the front lines of emergency veterinary care where every second counts. You'll handle a wide range of cases, from minor to complex emergencies, collaborating with our doctors and team in delivering gold standard, emotionally intelligent care. You'll jump into triage, treatments, diagnostics, anesthesia, monitoring, and critical care nursing, all while being a steady, compassionate presence for pet parents in their most stressful moments. This role is for someone who wants to do the work they were trained to do (and more), continue growing their skills, and help people and their pets when they need it most. WHAT YOU'LL DO • Treat every patient and person like they matter—because they do! You keep people and pets together and stay by their side through tests, treatments, and everything in between, • Jump into action, delivering nursing care, treatments, and emotional support in the most critical moments, from triage to discharge, • Bring the meds, the monitoring, and the mastery—from IVs and CRIs to blood transfusions and beyond, • Communicate like a teammate and think like a leader, helping shape plans and keep care flowing, • Be ready to perform CPR like the trained RECOVER rockstar you are (not only do we offer training, we have the highest number of RECOVER CPR professionals and instructors in the industry!), • All the techy stuff—from diagnostics and imaging to sample collection and placing tubes, • Anesthesia, even for our most fragile patients? You own the rhythm and flow of each shift, making sure patients get top-tier care and every VEGgie has what they need to thrive. Job Description RiseWell Community Services is seeking a full-time Employment Specialist to join the Clinical Department. This is a mobile position where travel is required between both Suffolk and Nassau County. The home office to report to would be West Babylon. In this role, you will utilize the Individual Placement and Support (IPS) Model for individuals enrolled in ACT and CORE services. Responsible for vocational and career assessment, benefits counseling, job development and placement, and job retention support for individuals with mental health diagnoses. Spend a minimum of 50% of work hours off-site, in the community, participating in job development activities.
