The second program provides 10 units of housing for SPMI individuals who reside in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs and adult homes. The third program provides 7 units of housing to individuals who are SPMI and have been residents of either OMH Psychiatric Centers or OMH operated residential programs for one year or longer. All apartments are located in Brooklyn. The program offers Case management services focusing on removing barriers to maintain housing by ensuring that all clients’ basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focus on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy. Clients contribute 30% of their income towards their rent. Position: Bilingual (Spanish) Case Manager Reports To: Program Manger Location: 1812 Vyse Avenue Bronx, NY 10460 What The Bilingual (Spanish) Case Manager Does: • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Conduct group sessions and/or workshops to assist tenants with independent living skills., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building), • May schedule appointments for tenant with referral organizations., • May assist tenants in completing applications for benefits and entitlements, and may process applications on tenants' behalf., • May follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • May evaluate actual living conditions of tenants through home visits. Minimum Education/Experience Required: • Bachelor's degree (e. g., B.A., B.S.W.) and two years of applicable experience. Other Requirements: • Obtain “Criminal History Checks” (CHC) fingerprinting through the New York State Office of Mental Health (OMH) and the Justice Center for the Protection of People with Special Needs (Justice System) and Staff Exclusion List (SEL) clearance., • Ability to maintain fingerprint clearance throughout the duration of employment., • Flexible hours may be needed occasionally to provide shift coverage until 8 pm. Compensation: $45,000 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.   Job Description Essential Job Functions: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Conducts client centered safety assessment and risk management planning. Provides crisis intervention and direct assistance to victims of crime and abuse; develop creative solutions reflecting clients’ unique needs in collaboration with client. • Advocates for clients for public entitlements with systems and other agencies, where necessary, such as obtaining orders of protection, help with hospital/medical related needs, Office of Victims Services compensation, and relocation assistance., • Provides information and referrals and facilitates linkages for clients for services such as shelters, mental health treatment, public assistance and other entitlements based on assessment and clients’ needs., • Assesses for and provides emergency practical assistance for clients including transportation, food vouchers, financial assistance and lock replacement in accordance with agency policy., • Engages in regular supervision meetings with supervisors to review cases, reflect on client interaction and build skills., • Maintains thorough and accurate case records and statistics in web-based case management system., • Collects client data, tests new interventions and tracks outcome. Participates in staff meetings, individual and group supervision, community meetings and relevant trainings and seminars. • Conducts educational presentations and trainings and participates in outreach events for community groups about intimate partner and family violence, victim assistance resources and other related topics., • Works collaboratively with other Safe Horizon programs to facilitate linkages for clients. Knowledge and Skills: Required: • Must be sensitive towards and understanding of victimization., • Must be able to demonstrate respect, compassion and a non-judgmental approach to victims of crime and abuse., • Must be willing to participate in a learning community., • Must be organized, detail oriented and solution focused., • Must demonstrate the ability to work independently and be resourceful., • Must demonstrate leadership skills (acts as a “go-to” person for colleagues, takes initiative; suggests ways to make processes work better)., • Must be a team player., • Possess strong communication and organizational skills., • Excellent interpersonal skills., • Able to work under pressure in a calm and reassuring manner. Education: • A bachelor’s degree in a related field (Social Work, Psychology, Human Services etc.) or equivalent experience is preferred. Experience: • At least one year of prior direct service experience in a social service setting is required. Experience working with survivors of intimate partner and family violence and other victims of crime preferred. • Bilingual (English/Spanish, French, Chinese, Arabic, Korean) preferred.
