The role will primarily be in Manhattan with one scheduled day in Purchase and possible intermittent coverage days in Purchase. Job Description Position Summary: The Home+ Case Manager plays a vital role in providing trauma-informed, survivor-centered case management for individuals impacted by IPV and GBV. This position supports survivors in navigating complex systems including housing, financial resources, legal services, and safety planning to empower them to remain safely housed or transition to stable housing. Rapidly after release into the program, the Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Clinical Case Coordinator assists participants to comply with supervision appointments and court appearances during the pendency of their legal case. In this role, you will conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. You will ensure participants receive services that are strengths-based, gender-specific, and culturally sensitive while adhering to the core principles of risk, need and responsivity to reduce recidivism. The goal is to increase engagement in community treatment services and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. This position is based in our Bronx Community Office and fieldwork in the larger NYC community as needed to assist participants.
