Job Description JOB DEFINITION: The Care Coordinator works with the CN Guidance Critical Time Intervention Team to deliver services within an evidence-based model that is time limited, phase-based care management approach focused on enhancing continuity of care during transitional times. Care Coordinators develop the person center plan, share knowledge of community resources, provide linkage to resources and engage individuals in skill building. We are looking for committed and energetic Behavior Intervention Specialists (BIS) who will conduct, assess and develop behavior support plans, address challenges and train our QIDPs and Direct Support Professionals (DSP) in the Day Habilitation Program. The salary range for this position is $52,000-$60,000 annually. This means working closely with our emergency doctors to provide same-day ultrasound scans for pets when they need it most. Job Description JOB DEFINITION: The Registered Nurse will work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches. The Registered Nurse provides direct patient care and implements treatment plans under the direction of the Nurse Manager and medical staff, following established clinical guidelines, policies, and procedures. EDUCATIONAL REQUIREMENTS: Must be currently licensed as a Registered Nurse by the New York State Department of Education and possess either an Associate's Degree (if awarded prior to December 18, 2017), Bachelor's Degree and/or Master's Degree in Nursing from an accredited program*.
