Job Description & Requirements • Specialty: Long Term Care, • Discipline: LPN / LVN, • Start Date: 12/22/2025, • Duration: 52 weeks, • 40 hours per week, • Shift: 8 hours, nights, • Employment Type: Local Contract Nights - FT - 5 x 8 Duties/Responsibilities Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse will be involved in the treatment of residents. • Provide care and service to a minimum of 20 and a maximum of 40 Veterans., • Supervise C.N.A in Resident Care., • Provide direct patient care, including bathing, dressing, grooming, and toileting assistance as needed., • Monitor residents' physical and emotional well-being, reporting any changes to the RN or physician., • Assist residents with mobility, transfers, and ambulation, ensuring safety and the use of assistive devices.Clinical Responsibilities, • Perform vital signs, weight checks, and assist C.N.A with ADL care such as feeding and weight measurement., • Administer medications (oral, topical, injectable, inhaled) as prescribed and document accurately., • Provide wound care, including dressing changes, skin assessments, and preventive skin care., • Collect specimens (urine, stool, sputum, blood via finger stick) for lab testing., • Monitor and report lab values and diagnostic results to the RN or physician., • Insert, maintain, and monitor urinary catheters, IVs (depending on state regulations), and feeding tubes., • Administer oxygen therapy and monitor residents on oxygen.Communication & Documentation, • Document all care provided, observations, and resident changes in the medical record promptly., • Communicate effectively with the RN, Providers, residents, and designated representatives within the scope of practice., • Report resident status changes or changes in condition immediately to the supervising RN or physician., • Participate in shift hand-off reports to ensure continuity of care.Supervision & Delegation, • Supervise and direct CNAs in daily resident care tasks., • Ensure CNAs follow care plans and report concerns., • Perform skin observation during showers and pre/post dialysis; report and document any abnormalities to the RN., • Provide feedback, support, and guidance to C.N.As.Care Planning, • Assist in reviewing and updating resident care plans under RN supervision., • Participate in care conferences with interdisciplinary team members., • Implement interventions in accordance with care plans and physician orders.Emergency Response, • Recognize and respond promptly to medical emergencies with RN prompt notification for assessment (falls, seizures, cardiac/respiratory distress)., • Assist in performing CPR and first aid as required., • Contact RN, physician, or emergency services when necessary.Resident Advocacy & Support, • Promote dignity, respect, and resident rights in all care activities., • Provide emotional support to residents and families., • Educate residents and families about health needs, medications, and care routines (within scope).

Essential Duties:  Coordinates recruitment and hiring process specifically for the Reflections Care Management team members  Completes staff performance appraisals/disciplinary action  Facilitates general orientation for all staff on Reflections Program  Conducts monthly in services on various dementia related topics for all staff  Responsible for environmental rounds to check for safety of exit doors, resident room safety and unit safety, and adherence to brand standards  Provides direct support to care management staff for behavior symptom management/resident care approaches  Coordinates and facilitates the RCP Training program for primary staff on unit. Responsible for tracking completion and scheduling of RCP classes.  Suggests residents for restorative potential and assures program is consistently carried out by staff to maintain resident's maximum practicable level of function.  Cooperate with other resident services for complete evaluation of care.  Assist in establishing linkages with and arranging for services from public and private sources for income, health, mental health and social services.  Assist the resident in making application for, and maintaining, income entitlements and public benefits.  Coordinate care planning meets per state regulations.  Coordinates, implements, and monitors all resident service plans with the Wellness Director.  Utilizes Brandywine standards and standards of professional organizations in conducting resident care.  Demonstrates knowledge and skills necessary to safely provide care appropriate to age of residents managed on unit.  Cooperates with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained.  Assist with Admits, transfers, and discharge residents as required.  Reports to the RN to assess and documents resident and family response to interventions.  Provides written and/or oral status reports of residents to assure necessary follow up actions.  Requisitions and arranges for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures.  Assists the physician on rounds and ensures that pertinent information is communicated to and from him/her.  Assists in resident rehabilitation activities. Working Conditions: Moves intermittently during working hours and subject to frequent interruptions. May be subject to hostile and emotionally upset residents, family members, personnel and visitors.
