We provide community-based prevention and family foster care services, group homes in the community for struggling children and families within the child welfare and juvenile justice systems, and short-term residential services for unaccompanied migrant children. PROGRAM BACKGROUND: The Medical Department provides medical services to children ages 0 to 21 that have been placed in MercyFirst care. Children placed in care are more likely than not to be exposed to adverse childhood experiences (ACE) and traumatic stress. ACE exposure is linked to poor academic performance, poor health outcomes, and certain diseases. The Medical Department is the first line of defense to help detect health problems early. They do so by providing evidence-based, strength-based, trauma-informed, and child-centered approach to service delivery. POSITION SUMMARY: Under the direction of the Nurse Supervisor, the Medical Assistant will assist the medical team in providing safe and knowledgeable medical assistance for youth in foster care and in residential settings. Responsible for general clerical tasks within the Medical Program as assigned by the supervisor. Job Description Hope For Youth's Residential Services Department provides community based, trauma informed, youth and family driven care to adolescents and young adults in Nassau and Suffolk County. Serving youth from varying points of entry with the child welfare system, juvenile justice system, and runaway and homeless youth/young adults, the opportunities to alter the trajectory for youth in need are abundant. Youth are supported 24/7 by a team of multi-disciplinary professionals, paraprofessionals, and agency administration that consistently meets standards for accreditation and licensing regulations. Hope For Youth's Adolescent Diagnostic and Respite Services Program ("Diagnostic") provides respite, emergency and diagnostic services for up to 12 youth, co-ed, from ages 12-20. Youth are referred through Nassau and Suffolk Department of Social Services, Probation and the Crisis Bed Network. The Diagnostic Program Manager will oversee all case management services, aftercare services, prepare psychosocial assessments and diagnostic evaluations. The Program Manger provides direct supervision to Assistant Program Manager, Case Worker and Case Worker Assistant. The Program Manager is instrumental in maintaining the overall functions of the program with the assistance of the Assistant Program Manager. The position offers the opportunity for some remote work within agency policy, including but not only completion of new hire introductory period and good standing. Key Responsibilities To be successful in this role, you must be ready, willing, and able to: • Function within the policies and procedures outlined in the Program Practice Guide and Employee Manual., • Provide overall management of your assigned site., • Complete reports, case documentation, and psychosocial assessments., • Liaise with DSS, schools, and collateral contacts., • Deliver crisis intervention and counseling when needed., • Supervise, coach, and provide staff with the knowledge and tools to succeed., • Develop discharge plans and provide 6 months of aftercare services., • Conduct home visits as necessary., • 50% General casework, documentation, and reports, • 20% Supporting youth, • 20% Supporting staff, • LMSW required; LCSW preferred, • 2 years' post-master's experience with child-serving systems (child welfare, juvenile justice, etc.), • Strong computer proficiency (Microsoft Word, Excel, and reporting portals), • Valid NYS driver's license in good standing, • Access to a reliable vehicle, • Ability to provide 24/7 on-call availability during the week; participate in weekend on-call rotation Physical Requirements • Operate a motor vehicle carrying up to 10 children/youth, • Lift/move up to 50 lbs., • Escort children/youth into the community (including stairs if necessary), • Restrain a child/youth during a crisis, when required, • Regular walking/standing, bending, kneeling, climbing stairs, reaching overhead, and repetitive wrist/finger movements, • Ability to read/write reports and log entries This opportunity is being offered as Full Time 35/hr. workweek Only candidates selected for an interview will be contacted.
