About the PINS Diversion Program: Our Person in Need of Supervision (PINS) Diversion Program aims to prevent court involvement and out-of-home placements for youth under 18 by providing short-term, family-centered, in-home support throughout Nassau County. We focus on building problem-solving skills and linking families to long-term community resources. Your Role as a Case Planner: As a Case Planner, you will support youth and families by improving family dynamics, encouraging behavioral change, and reducing the risk of placement outside the home. This role requires flexible hours, including evenings and weekends, and a field-based presence using your own vehicle. Key Responsibilities: • Carry a caseload of 15–25 families, • Conduct weekly in-home visits with youth and families, • Deliver life coaching, parent skills training, and case management services, • Attend school meetings, court appearances, and service planning sessions, • Complete and update CANS-NY assessments, • Maintain accurate, timely documentation and progress reports, • Participate in team meetings, supervision, and ongoing staff development, • Adhere to HFY and regulatory standards and practicesQualifications:, • BSW or Bachelor's in a related field with 2+ years of casework experience, or MSW, • Strong written, verbal, and tech skills, • Bilingual (Spanish/English) preferred, • Valid NYS Driver's License & access to reliable transportation, • Ability to work a flexible schedule to accommodate families, • A mission-driven team dedicated to helping Long Island families Ongoing training and professional development * Hybrid work model and supportive supervision * Eligible for comprehensive benefits, including medical, dental, vision, and retirement plansApply Now If you're ready to make a difference in the lives of youth and their families, we'd love to hear from you!
