Job Summary Hourly Rate - $18.00 The Habilitation Specialist (DSP), is responsible for assisting the people we support, while following established treatment plans. QSAC’s Residential program emphasizes the importance of promoting independence through the development of daily living/social skills and integration into the community through recreational outings. You will abide by the Direct Support Professional Code of Ethics at all times. Shift Information: Flexible Schedule - Mornings, Afternoons, Evenings, Overnights - Weekdays and/or Weekends. May be required to assist with overnight outings. TSINY is looking for a Part-Time Case Manager for one of our mental health residential programs located in Bellerose (Queens), NY. Salary: $26/hr., Part-Time Valid Driver License Required Position Summary The Case Manager coordinates and monitors the assessment, treatment planning and care coordination for each resident on their caseload. The Case Manager serves as an advocate and intermediary for and on behalf of residents’ rights. Case Management staff will work in partnership with our residents to establish goals, find appropriate supports and access independent community housing.
