Job Description JOB DEFINITION: The mission of the CN Guidance FACT Team is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. The FACT Team provides an integrated set of evidenced-based treatment and support services through a highly individualized approach. Services are provided in the community in places and situations where problems arise. EDUCATIONAL REQUIREMENTS: Bachelor's Degree in social work, psychology, rehabilitation counseling or related field required. EXPERIENCE REQUIRED: One year experience working in a mental health setting at the Masters or higher level or two years' experience working in a mental health setting at the Bachelors level.
