Position Summary The License Practical Nurse (LPN) works under the direct supervision of the Registered Nurse and the Regional Practice Director to support the delivery of high-quality patient care. Position Summary Reporting to the Pharmacy Coordinator, the Pharmacist will provide safe and effective pharmacy services in addition to dispensing medications. These services include the provision of pharmaceutical care, medication dispensing, patient counseling, drug information services and patient and staff education. Responsibilities • Provide patient education on medication and reason for use, frequency and dosages, side effects, and desired therapeutic effects. Enhance educational experience using drug information flyers or pamphlets appropriate for patient level of health literacy., • Fills in-house generated prescriptions in accordance with all federal regulations and all state regulations as per the NJ Board of Pharmacy., • Fills outside provider prescriptions in accordance to OHI Outside Prescription Policy in addition to all federal and state regulations as per the NJ Board of Pharmacy., • Serves as a resource of information on medications, pharmacokinetics and interactions, to providers., • Assist providers in the management of patients with polypharmacy including conducting medication reconciliation, writing medication lists, completing the patient medication card, and providing other means for patients to understand how to take their medications., • Works in the CEC periodically to assist providers with requests from pharmacies regarding therapeutic interchanges, questions regarding prescriptions and continuity of care to ensure patients get prescriptions in a timely manner., • Work collaboratively with the Coumadin Management Team to evaluate, assess and assist patients with their INR once notified of results by nurse.
