Job Description RiseWell Community Services is seeking a Full Time Case Management Administrator To join the Suffolk County, Department of Health, Division of Mental Hygiene, ADULT SPOA Unit REPORTS TO: Kristen Crispino, LCSW, Director of Adult Services Suffolk County Division of Mental Hygiene HOURS: Monday-Friday, 9am to 5pm QUALIFICATIONS: Bachelor’s degree, or related experience required JOB FUNCTION: The position in the Adult SPOA is an administrative position with a clinical component. As a team member of the SPOA unit you would be processing applications for adults with mental illness seeking an array of services, including but not limited to: Assisted Outpatient Treatment, Case management and ACT services, SPA Mental Health Housing, and/or Crisis Respite housing. This requires thorough review of the application and supporting documentation; investigation of documents; case presentations; general knowledge regarding Health Homes and resources in the mental health system; general knowledge of mental health housing in Suffolk County; ensuring court-ordered Mental Health Services are in place and that the person under an AOT order is compliant with those services. In addition to linking individuals to needed services, you will also have a caseload of individuals under an active AOT court mandate, which you will be responsible for receiving, documenting and following up on weekly updates from current providers, as well as requesting and following up on additional documentation as required. This is a very intensive program, where the work can, at times, take on an emergent need. You will need to be organized and able to complete tasks timely and efficiently. It is necessary that you possess good communication and negations skills, and feel comfortable interfacing with agency directors, psychiatrists, hospital social workers, police, and at times attorneys and criminal justice workers. You are working as part of a team that monitors hundreds of patients and is responsible to connect people to services timely. The ability to function as part of a team is essential.
