Job Description RiseWell Community Services is looking to hire a Full-TimeTeam-Leader to join the Assertive Community Treatment (ACT) Teams. In this role, Supervise and provide direct services on a mobile, multi-disciplinary treatment team for persons with severe and persistent mental illness. Qualifications: Master’s degree or higher with license in social work or Mental Health Counseling, psychology, rehabilitation counseling (if rehabilitation counselor, C.R.C. preferred), OR licensure/registration as registered nurse (RNs are encouraged to be masters level or nurse practitioner), or physician. License required (LMSW, LMHC, LCSW, LMFT). A valid and clean NY State driver license. Minimum 3 years of experience in a treatment or rehabilitation setting.
