Job Responsibilities/Essential Functions \n\n • Utilizes the nursing process effectively (assessing the patient and situation, identifying the problem, planning, implementing, and evaluating care through the health care continuum).\n, • Consistently demonstrates competent clinical skills. Coordinates and completes patient assessment and re-assessment to positively impact on quality outcomes.\n, • Assesses barriers to learning and functional status of patient. Reviews and appropriately responds to patient messages, in collaboration with clinician(s).\n, • Recognizes emergency situations and acts appropriately. Clarifies physician orders as indicated.\n, • Maintains a safe environment for team members and patients/visitors: physical, psychological, and environmental. Utilizes the HRO safety principles.\n, • Reports all injuries (patient or staff) using event reporting system.\n, • Demonstrates knowledge of medication and vaccine administration according to policy, carries out physician orders in an accurate and timely manner.\n, • Monitors effects of medications and reports adverse effects in a timely manner.\n, • Demonstrates knowledge of policies and procedures, including but not limited to knowledge and skill of emergency equipment and procedures.\n, • Identifies and reports all risk management issues in a timely manner.\n, • Responsible for phone management of patients.\n, • Triages phone calls utilizing critical thinking and assessment skills to evaluate the chief complaint and urgency of the problem. Triages phone calls to determine acuity to assist in scheduling appointments.\n, • Provides comprehensive medical record documentation. Documents all communications and maintains patient confidentiality.\n, • Participates in and/or organizes regular practice huddles, communicates effectively with team members and physicians; actively listens and seeks feedback. Collaborates with team members to solve problems and resolve conflicts. Functions as a resource/mentor and provides support to team members to resolve patient care and team concerns/issues.\n, • Utilizes standard precautions including, appropriate isolation techniques. Consistently follows the Ambulatory Health Care National Patient Safety Goals.\n, • Through the model of Shared Governance, collaborates with all members of the health care team with a focus on promoting evidence-based care within a healing culture for the patient population served and their families.\n, • Demonstrates knowledge of documentation guidelines. Documents patient’s response to the ordered treatment protocol. Reports changes in a patient condition to the clinician immediately. Consistently collects and completes all required documentation. \n accuracy of order entry and transcription. \n • Maintains competency in use of all needed information systems. Able to communicate effectively verbally and electronically.\n, • Pursues and participates in formal and independent learning activities to enhance knowledge, clinical competency, and professional development.\n, • Monitors quality dashboards for assigned clinicians/practice.\n, • Performs other duties per operational needs of the assigned clinical practice area and within the scope of RN license.\n, • Supports the overall strategic goals of the clinical practice care area, AMG, the AHS hospital sites, Atlantic Visiting Nurse, and the system.\n, • Exemplifies and fosters the AHS P.R.I.D.E. values in their clinical practice: Purpose, Respect, Innovation, Diversity, and Inclusion, and Extraordinary Caring.\n\n Education Requirements \n\n • High School Diploma/GED; graduate of an accredited School of Nursing \n required\n • Bachelor’s degree in nursing preferred\n\n \n Experience Requirements \n\n • At least 2 years’ clinical practice, ambulatory practice preferred\n\n Certificates Required \n\n • American Heart Association Basic Life Support (BLS) credential required\n\n \n Licenses Required \n\n • RN license, valid in the State of N.J.\n\n \n Other Desired Skills \n\n • Knowledge and command of electronic medical records systems.
