These youth may include: * Persons In Need of Supervision (PINS) * Juvenile Delinquents (JD) * Juvenile Offenders (JO) * JD–Raise the Age * Adolescent Offenders Our program focuses on family-centered treatment services that have proven effective in reducing delinquency risk and avoiding out-of-home placement. Position Overview: The Case Planner plays a critical role in supporting youth and families by improving individual functioning and family dynamics. This includes timely and accurate submission of monthly Medicaid Billing Reports to the Billing Dpt., • Oversee all medication monitoring procedures and assume any medication management related tasks and responsibilities in the absence of a Senior Counselor. Housing Supervisors are to review the medication logs/EMAR on a daily basis to ensure all procedures are being followed appropriately., • Oversight of the sites to ensure a clean, safe and homelike environment. Ensure proper maintenance and upkeep of the site in collaboration with the facilities and management team., • Ensure all residents are treated with dignity and respect and are given the opportunity to actively participate in their service and support plans in order to provide collaborative treatment panning., • Organize and encourage respectful interactions between residents, conduct house meetings and offer supports and training as needed to residents in the program., • Conduct intakes for potential residents in cooperation with the admissions committee., • Review and approve all time off requests, time sheets and travel expenses for staff within 48 hours of submission., • Oversee program finances, including weekly client distributions, petty cash, fee payments, check requests, food and supply budgeting and ensuring appropriate funding and benefits are in place for all residents., • Oversight of recreational activities., • Serve as a liaison with outside service providers and advocate for the needs of residents in the program, when necessary., • Ensures that appropriate discharge planning activities occur for all residents., • Responsible for routine oversight and monitoring of all client records to ensure that records are up to date and include all required documentation in compliance with applicable individual program regulations and policy manuals., • Provide direction and support to staff during crisis situations.
