Job Description Hope For Youth's Residential Services Department provides community based, trauma informed, youth and family driven care to adolescents and young adults in Nassau and Suffolk County. Serving youth from varying points of entry with the child welfare system, juvenile justice system, and runaway and homeless youth/young adults, the opportunities to alter the trajectory for youth in need are abundant. Youth are supported 24/7 by a team of multi-disciplinary professionals, paraprofessionals, and agency administration that consistently meets standards for accreditation and licensing regulations. Hope For Youth's Adolescent Diagnostic and Respite Services Program ("Diagnostic") provides respite, emergency and diagnostic services for up to 12 youth, co-ed, from ages 12-20. Youth are referred through Nassau and Suffolk Department of Social Services, Probation and the Crisis Bed Network. The Diagnostic Program Manager will oversee all case management services, aftercare services, prepare psychosocial assessments and diagnostic evaluations. The Program Manger provides direct supervision to Assistant Program Manager, Case Worker and Case Worker Assistant. The Program Manager is instrumental in maintaining the overall functions of the program with the assistance of the Assistant Program Manager. SCHEDULE Flexible schedule SUMMARY Family Service League is seeking a Fee For Service Social Worker for our Mental Health Clinic in Huntington. The Social Worker will provide evaluations, treatment planning, psychotherapy and crisis intervention to children, families and adults with a full range of DSM diagnoses in a clinic setting, including co-occurring disorders. Services are delivered in a trauma informed, culturally and linguistically competent manner. This position is currently a hybrid of onsite and remote work, providing both in-person and telehealth services. RESPONSIBILITIES • The Social Worker will provide evaluation/assessment, treatment planning, psychotherapy and crisis intervention to those with behavioral health needs., • Provide individual and family therapy to children ages 5 and up, adolescents and adults with a full range of DSM diagnoses., • Provide services that are delivered in a trauma informed, culturally and linguistically competent manner., • The Social Worker will provide crisis intervention during scheduled work hours as is needed., • Complete all clinical documentation in accordance with OMH standards in the electronic medical record., • Collaborate with family and other collateral contacts including care coordination providers., • The Social Worker will participate in routing supervision and team meetings., • Attend agency mandated trainings., • Provide telehealth when needed., • Attend routine supervision and team meetings., • Identify and recommend additional referrals as needed., • Computer proficiency, including Microsoft Office, required, • All other duties as assigned.
