Job Description JOB DEFINITION: The Registered Nurse will work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches. The Registered Nurse provides direct patient care and implements treatment plans under the direction of the Nurse Manager and medical staff, following established clinical guidelines, policies, and procedures. EDUCATIONAL REQUIREMENTS: Must be currently licensed as a Registered Nurse by the New York State Department of Education and possess either an Associate's Degree (if awarded prior to December 18, 2017), Bachelor's Degree and/or Master's Degree in Nursing from an accredited program*. This education effort is not limited to home visits but may also include the coordination of various groups to include but not limited to, medication, wellness, etc., • Monitors the client's medication regimen to ensure completeness and accuracy in the provision of care., • Assists the psychiatrist by scheduling clients for visits based upon client needs, s/p hospitalization and/or incarceration. The RN will also advise the psychiatrist of all medication renewal needs., • Must be available (along with other members of the Forensic ACT Team) on an as needed basis twenty-four (24) hours a day, seven (7) days a week, including responsibility for periodic pager coverage., • Responsible for making a minimum of four (4) to six (6) client visits daily. These visits are to be chosen at the beginning of each work day and identified on the client work board in the bullpen by the RN's initials., • Responsible for entering the documentation for each client visit in the IMA System as instructed by the team leader within twenty-four (24) hours after the client visit.
