Job Description Director of Reflections/Memory Care Coordinator We are seeking a caring, dedicated and experienced LPN nurse supervisor to become the Director of Reflections, our dedicated Alzheimer’s and dementia Memory Care Neighborhood at our beautiful and luxurious community of The Bristal Assisted Living at North Hills. In this crucial role, you will be responsible for the day-to-day management and success of the Reflections Program, including planning, organizing and coordinating special needs programs. You will also provide team member supervision and training, and act as a liaison and advocate for our residents and their families. This is a hands-on position requiring occasional participation in assisting residents with ADLs. Schedule: Sunday- Thursday 9am-5:30pm Additional responsibilities include: • Ensuring that resident care and management needs are sufficiently and appropriately assessed, • Serving as the liaison between residents, the resident’s legal representatives, their families, administrative staff and community-based service providers, • Motivating and developing team members, including providing in-service education of the special needs program to Reflections area team members on health/wellness related topics, • Leading the daily operations of the Reflections Memory Care area and ensuring compliance with company and Department of Health (DOH) regulatory policies and standards regarding resident admission and discharge procedure as well as compiling appropriate medical case notes and other required documentation, • Scheduling and overseeing team members and assisting with medication management Full-time schedule requires working one weekend day per week (Tues-Sat or Sun-Thurs).
