RESPONSIBILITIES • The Case Manager will engage with clients who have a primary history of mental illness., • Develop strengths assessment with clients., • Offer crisis and safety planning, in collaboration with mental health providers., • Create individualized planning with the clients in the areas of health, education, mental health, vocation, socialization, benefits, legal and life skills., • Meet a minimum of 2 times per month to support the youth with their goal achievements., • The Case Manager will assist in the coordination of services with providers, benefit systems and the community., • Assist in the linkage to service providers, benefit systems and the community., • Advocate for clients as needed., • Help clients navigate service systems so they get the support that they need in their recovery., • The Case Manager will initiate crisis intervention with linkage to appropriate mental health or health services, as necessary., • Report all client incidents and accidents., • Inform supervisor when other providers are not collaborating in the effort to achieve services for the client or have plans to inappropriately discharge a client from housing., • Follow guidelines set by the Office of Mental Health and Division of Mental Hygiene SPOA unit Risk Management., • All other duties as assigned., • Travel is required between worksites and client homes. QUALIFICATIONS Bachelor’s Degree in Social Work with 2 years of related experience or Master’s Degree with 1 year of related experience required. Excellent interpersonal and organizational skills required. Strong verbal and written communication skills required. Ability to solve problems and work as part of a team required. Proficient computer skills, including Microsoft Office, required. Valid and clean NYS Driver’s License required. PHYSICAL REQUIREMENTS This position requires sitting for long periods of time and travelling to conduct home visits.
