Community Oriented Recovery and Empowerment (CORE) services is a program serving individuals living with severe and persistent mental illness. This program serves Medicaid HARP enrollees and works to assist individuals in reaching their recovery goals. Services provided include Psychosocial Rehabilitation, which incorporates education and employment goals, and Peer Support. There is an extensive and broad range of services that include but are not limited to the improvement of daily living skills, managing finances, obtaining or improving safe and affordable housing, furthering one’s education, obtaining employment, achieving personal recovery and/or resilience, improving family and interpersonal relationships, preventing social isolation, connecting with effective and comprehensive behavioral health and primary care, and full community integration. The CORE Services Certified Peer Specialist is responsible for delivering Peer Support as well as the other direct care services offered within the CORE program. The CORE Services Certified Peer Specialist is responsible for ensuring members receive person-centered, trauma-informed and culturally competent services with a rehabilitation and recovery focus in their homes or their community. This role is responsible for facilitating the utilization of natural resources and the enhancement of recovery-oriented principles with the intention to assist individuals with initiating recovery, maintaining health and wellness, and enhancing the quality of personal and family life for long-term recovery. The Attorney provides legal support and guidance for all agency advocates. Legal representation in court includes orders of protection, child custody modifications, legal guardianships, UVISAs and can include an advisor of choice for College Title IX hearings, landlord tenant and other legal issues affecting victims and their rights. There is an opportunity for an attorney who has a long term goal of being involved in litigation as we explore the expansion of the legal services available to crime victims. The Case Manager will be responsible for conducting client assessments, developing Independent Living Plans, linking clients with community-based service. RESPONSIBILITIES • The Case Manager will complete new client intakes., • Meet with clients at a minimum of once per week for Face-to-Face meetings., • Develop Independent Living Plan for clients., • Work with clients in creating a budget to identify needs and obligations. Where applicable, incorporate upcoming shelter obligations., • Maintain and update the resource database of social service organizations relevant to the needs of the clients., • Provide ongoing support and counseling for clients in reaching their goals., • Provide assessment and crisis intervention., • The Case Manager will foster inter-agency collaboration to assist in client long-term stability and advocate for client needs., • Work collaboratively with members of a larger treatment or service coordination team, both within the agency and in the larger community., • Maintain up-to-date documentation regarding client services in the client files., • The Case Manager will collect client housing logs weekly. Complete monthly reporting in a timely manner. Submit documentation to DSS Housing Liaison., • Attend scheduled Case Management team meetings, individual supervision, staff meetings and trainings, as required., • Work in partnership with the Division Director and seek supervisory input, as needed., • Ensure rules, regulations and policies of shelter and model professional behavior., • Maintain and update resource database of social service organizations relevant to the needs of target population., • Set and observe appropriate boundaries with clients; observe client confidentiality and HIPPA protocols., • Establish and maintain positive working relationships with others both internally and externally to achieve the goals of the organization., • All other duties as assigned. QUALIFICATIONS Bachelor’s degree in Human Services preferred. Minimum of 1 year of experience in human services or related field required. Knowledge of the Suffolk County Department of Social Services preferred. Knowledge of human services and previous experience working with at-risk population preferred. Excellent interpersonal, verbal, and written communication skills required. Proficient computer skills, including Microsoft Office required; experience with HMIS, Google Apps, and web-based project management software preferred. Bilingual in Spanish preferred.
