Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming a CORE Peer Specialist. Community Oriented Recovery and Empowerment (CORE) services is a program serving individuals living with severe and persistent mental illness. This program serves Medicaid HARP enrollees and works to assist individuals in reaching their recovery goals. Services provided include Psychosocial Rehabilitation, which incorporates education and employment goals, and Peer Support. There is an extensive and broad range of services that include but are not limited to the improvement of daily living skills, managing finances, obtaining or improving safe and affordable housing, furthering one’s education, obtaining employment, achieving personal recovery and/or resilience, improving family and interpersonal relationships, preventing social isolation, connecting with effective and comprehensive behavioral health and primary care, and full community integration. The CORE Services Certified Peer Specialist is responsible for delivering Peer Support as well as the other direct care services offered within the CORE program. The CORE Services Certified Peer Specialist is responsible for ensuring members receive person-centered, trauma-informed and culturally competent services with a rehabilitation and recovery focus in their homes or their community. This role is responsible for facilitating the utilization of natural resources and the enhancement of recovery-oriented principles with the intention to assist individuals with initiating recovery, maintaining health and wellness, and enhancing the quality of personal and family life for long-term recovery. Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. The Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. They hold primary responsibility for developing program goals and methodologies and supporting DSPs to correctly implement and document such activities. The Client Coordinator ensures integrated services by fostering team awareness of service integration, coordinating service delivery with day program services, monitoring service provision by agency clinical staff, consultants, and community vendors, and ensuring services align with IDT treatment recommendations. They ensure a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to maintain adequate coverage levels and ensure an atmosphere conducive to development and well-being. They document progress and update Plans of Care / ISPs according to agency procedures. The Client Coordinator reports regularly to the Supervisor regarding program compliance, noncompliance, and remedial actions. They identify needs and request support as necessary. They participate in clinical reviews, incident reviews, human rights committee meetings, and other meetings as directed. As schedule allows, the Client Coordinator serves as primary chair for Life Plan meetings. They review documentation and provide technical assistance to program management and staff. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES The employee must be able to interact with individuals and staff professionally and ethically, understand and implement service plans, and complete required documentation clearly, legibly, and on time. The Care Manager is responsible for providing and coordinating services for their assigned caseload.
