The Clinical Case Management Specialist will ensure a smooth transition for Adult Home Residents moving into Supported Housing via the Impacted Adult Home Initiative//Settlement. Attend care coordination meetings, collaborate with ICL in-reach staff and external care coordinators, assess consumer needs, and discuss/advocate for needed services. Maintain adherence with settlement agreements regarding consumers moving into ICL units.
