Function: This position is co-located at the People USA Stabilization Center and acts as a core member of an interdisciplinary stabilization team for community members who are seeking crisis support. Responsibilities include but are not limited to crisis de-escalation, intake; diagnostic assessment and evaluation; treatment planning; brief intervention therapy; maintenance of case records; community education; coordination of treatment with other agencies and professionals providing ancillary services; and participation in client discharge planning. This is a contracted position working 37.5 hours/week, with most hours at the Ulster County Crisis Stabilization Center run by People USA. Job Description Overview MHA in Ulster County, Inc. has full-time (37.5 hours per week) Residential Coordinator positions available at our TTAP and Locust St locations. The Residential Coordinator assists in the planning and implementation of a rehabilitative, high expectancy program, and individually designed service plans for the adult population with mental illness and secondary problems in substance abuse, residing in one of MHA's residential programs. These services will be individualized and client centered to enhance the functioning levels and quality of life of the client population. The position provides case management services to adults living in our residential program and teaches life skills that prepares them to live more independently in the community. The available full-time schedule is Monday - Friday with every other weekend requirement: 3pm-11pm The pay rate for this position is $19.70 hourly. There is an additional pay differential for the evening schedule. Job DescriptionDescription: Working with medical providers to coordinate all aspects of the Medication Only program and necessary documentation and chart maintenance for all medical providers. Ability to multitask and prioritize is a must. Adherence to HIPAA regulations a must. Responsibilities: · Assist with obtaining paperwork on clients-labs, physicals, discharge summaries as needed. · Update and maintain patients’ health record. · Assist with obtaining consents from clients. · Use Electronic Health Records to support all transactions. · Answer patient's questions and ensure quality customer service. · Collaborate with doctors and nurses to help with obtaining necessary paperwork for appointments. · Use data resouces for tracking chart completion. · Communicating with client pharmacies and Primary Care Providers. · Coordinates the transition of clients from the OPC Medication Only Program and from the Med Only back to OPC. · Send Med Only letters to newly enrolled clients. · Check on Med Only clients monthly (if no med apt). · Keep an updated census for all clinics for the clients enrolled in the Medication Only Program. · Keep track of the status of Med Only Treatments Plans. · Send reminder emails of TPR due dates to the medical providers. · Keep track of discharge summaries. · Send closing letters and discharge summaries to clients. · Keep track of safety plans and HQA’s and send status to clinic nurses. · Check on High Risk cases. · Keep track of the documentation/consents for the Med Only clients. · Work with Nurses to ensure medication refills happen smoothly. · Set up follow-up appointments with medical providers.
