PURPOSE AND SCOPE: Manages patient care in home therapies programs while maintaining cost-effective clinical operations. Provides direction and guidance to the interdisciplinary team providing care to the Home Therapies patients to ensure that the highest standard of care is provided. Collaborates with the Business Unit management team to grow the assigned Home Therapies programs. PRINCIPAL DUTIES AND RESPONSIBILITIES: • Manages the operations of multiple Home Therapies programs with direct responsibility for results, including growth, patient retention, program costs, operational methods, scheduling, and staffing., • Directs activities related to the area’s strategic plans, goals and objectives ensuring alignment with those of the region, division, and company. Implements quality goals and action plans to achieve quality standards., • Collaborates with operations leaders, market development, in-center leadership and patient education and quality teams to promote home therapies growth in assigned programs., • Responsible for profit and loss management of the assigned program including optimal performance of program operations to achieve or exceed budgets and key performance indicators., • Directs and manages all aspects of patient care for Home Therapies patients in assigned programs from admission through discharge of the patient. Provides direction, guidance, and support to staff members to ensure the safe and effective delivery of home therapy support care to patients with End-Stage Renal Disease (ESRD)., • Oversees patient and caregiver readiness and ability to perform dialysis treatments at home and assesses environment for suitability in the ongoing delivery of home dialysis therapy, including training patients and caregivers on the safe and effective operation and preventative maintenance of equipment., • Manages patient experience efforts by supporting company patient satisfaction initiatives, embodying the company values and patient experience drivers while encouraging others to do the same., • Ensures competency of staff in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone, maintaining appropriate staff licensures and CPR certifications., • Responsible for management of program supplies, equipment tracking, ensures that orders are replenished in a consistent and timely manner., • Directs activities related to the Continuous Quality Assurance Improvement, patient retention and Clinical Algorithm Adoption, patient access management and patient training success., • Directs data entry related activities such as patient admission, regulatory reporting, billing, physician orders, lab results, vital signs, patient status and any other treatment related data into systems., • Ensures regulatory, compliance, and audit activities are accomplished on time., • Orients and supports all new home therapies staff and provides ongoing education, ensuring compliance with all risk management initiatives, including in-center staff., • Responsible for hiring, coaching, and counseling employees, including performance reviews, disciplinary action, and terminations., • Provides leadership, coaching, and development plans for all direct reports to maintain an engaged and productive workforce, partnering with Human Resources on employee matters. Position Overview We are seeking an experienced and dedicated Advanced Practice Registered Nurse (APRN) to join our multidisciplinary medical team in Galloway, NJ. The APRN plays a key clinical role in delivering psychiatric and medical services to individuals in detox and residential treatment. This role requires clinical expertise in psychiatric evaluations, medication management, and a collaborative approach to patient care. Requirements Key Responsibilities • Provide advanced nursing expertise for the design and execution of medical and clinical activities in accordance with each patient’s treatment plan., • Conduct psychiatric and physical assessments, medication management, and consultations within scope of APRN practice., • Assess and assure the appropriateness of patient admissions and ongoing care., • Provide after-hours on-call coverage for emergency psychiatric or medical needs., • Attend and contribute to interdisciplinary team meetings to coordinate patient care., • Deliver in-service training to staff as needed., • Participate in facility performance improvement and utilization review activities., • Conduct peer reviews and contribute to the development and evaluation of policies and procedures., • Assist in developing a network of community resources for post-discharge continuity of care., • Collaborate with executive and clinical leadership on policies related to patient care., • Ensure timely and accurate documentation of assessments, orders, progress notes, and discharge summaries., • Provide consultation to patients and their families as appropriate., • Maintain a current collaborative agreement and prescriptive authority with the Medical Director., • Provide on-site services according to facility schedule and agreement., • Support and comply with infection control standards and collaborate with the Infection Control Nurse., • Ensure compliance with state and federal regulations and Joint Commission standards., • Uphold all safety, risk management, and workplace policies., • Licensed to practice as an APRN in the state of New Jersey., • Active NPI and DEA registration (X-Waiver preferred but not required)., • Current CPR certification., • Maintains all state-required certifications and licenses., • Minimum of 2 years’ experience in psychiatric or addiction treatment settings preferred., • Essential Knowledge, Skills & Attributes, • Proficiency in verbal and written communication., • Strong knowledge of addiction, dual-diagnosis treatment, and trauma-informed care., • Adherence to state Board of Nursing Scope of Practice and professional Code of Ethics., • Understanding of Infection Control protocols and Joint Commission standards., • Familiarity with HIPAA, Drug-Free Workplace, Workplace Violence Prevention, and Corporate Compliance., • High competency in electronic health records (EHR) systems., • Cultural sensitivity and ethical approach to diverse patient populations. Assist clients in obtaining employment by linking clients with jobs and educational/training opportunities and the development of community contacts and resources for jobs and training. Establish and maintain contacts with employers and other groups potentially interested in hiring clients as well as other community-based organizations promoting job development. Conduct career counseling and vocational assessments, visit all work sites, and conduct job readiness and job retention classes. Scope Of Duties: • Develop working relationships with prospective employers and employment opportunities based on the local labor market, demand occupations, and resident interests, skills, and aptitude., • Market clients to local employers and coordinate efforts with the Department of Labor and other employment services., • Link client to vocational training opportunities through local vocational/technical schools and the county employment and training programs., • Document client employment and progress in client's permanent file, and collects data as required by funding sources., • Conduct career counseling and vocational assessments for clients in the program within two-week orientation period., • Conduct job readiness and job retention classes on a monthly basis at a minimum or as needed in program., • Coordinate quarterly job fairs and monthly on-site recruitments with prospective employers for program participants., • Supervise job coaches where applicable., • Conduct drug and alcohol screens as directed or needed and report results to supervisor., • Verify all client employment in accordance with funding source requirements., • Visit all work sites within two weeks of client beginning employment., • Place clients in employment or vocational training prior to discharge from program., • Participate in required internal/external Treatment Team meetings., • Any duties assigned by Supervisor., • Interact with clients, staff, and outside contacts with respect and courtesy., • Maintain client and staff confidentiality in compliance with administrative policies and procedures. 1.
