This education effort is not limited to home visits but may also include the coordination of various groups to include but not limited to, medication, wellness, etc., • Monitors the client's medication regimen to ensure completeness and accuracy in the provision of care., • Assists the psychiatrist by scheduling clients for visits based upon client needs, s/p hospitalization and/or incarceration. The RN will also advise the psychiatrist of all medication renewal needs., • Must be available (along with other members of the Forensic ACT Team) on an as needed basis twenty-four (24) hours a day, seven (7) days a week, including responsibility for periodic pager coverage., • Responsible for making a minimum of four (4) to six (6) client visits daily. These visits are to be chosen at the beginning of each work day and identified on the client work board in the bullpen by the RN's initials., • Responsible for entering the documentation for each client visit in the IMA System as instructed by the team leader within twenty-four (24) hours after the client visit. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress, • Complete QI Training during onboarding and participate in QI refresher training as needed., • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
