Conducts comprehensive assessment of patients by determining patient needs, develops courses of action including treatment plan and outcomes. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health. \n \n Core Responsibilities: \n\n • Provision of exceptional primary care.\n, • Conduct office visits for routine and acute issues.\n, • Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.\n, • Care coordination with other providers, specialists, testing facilities, and agencies.\n, • Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)\n, • Assisting the care team with phone triage and outreach.\n, • Educating patients on their health conditions, care plans, and treatments.\n, • Participating in Oak Street Health promotional activities.\n, • Conducting home visits as needed.\n, • Other duties, as assigned.\n\n This role reports to the Center Medical Director and works closely with operational leadership. \n \n \n Required Qualifications: \n \n\n • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate\n, • National certification in at least one of the following specialties:\n \n, • Family Nurse Practitioner\n, • Adult-Gerontology Primary Care Nurse Practitioner\n, • Adult Nurse Practitioner\n, • Gerontological Nurse Practitioner\n \n, • Active, non-probationary state Nurse Practitioner license\n, • Active DEA license\n, • US Work Authorization\n\n Preferred Qualifications: \n\n • Experience in primary care: internal medicine, geriatrics, or family medicine\n, • Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).\n, • Passion for teamwork and the opportunity to collaborate cross-functionally\n, • Desires to be a part of an innovative model focused on empirically-guided population health\n, • Bilingual proficiency in applicable areas\n\n \n \n \n \n Anticipated Weekly Hours 40\n \n \n Time Type Full time\n \n \n Pay Range \n The typical pay range for this role is: \n $104,442.00 - $224,952.00\n \n This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
