Job description POSITION OVERVIEW: The Senior Case Manager will lead and support shelter case management services that maximize safety and connect residents to needed services and support. CAMBA’s Cure Violence Program works with 16 – 24 year old at risk youths who are or may have potentially been involved in street violence, community residents, businesses and community-based organizations to implement Cure Violence: The Campaign to Stop the Shooting. Cure Violence is a public health strategy to reduce and prevent shootings and killings. This program will focus on a defined area identified as a “hot spot” in Brownsville, Brooklyn for shooting violence within the 73rd Precinct. The program works to prevent gun violence by: 1) mediating conflicts that may end in gun violence, 2) changing the thinking of individuals who are most at risk of being victims or perpetrators of violence, and 3) working closely with a broad coalition of local partners to promote a visible community-wide message that shooting is unacceptable behavior. Save Our Streets South Bronx is a replication of the Chicago Project for Violence Prevention’s evidence-based Cure Violence program, a cutting-edge gun violence prevention model that aims to reduce and prevent shootings through the use of public health strategies. The Bronx Community Justice Center is seeking an Outreach Worker for the Save Our Streets South Bronx program. Reporting to the Outreach Worker Supervisor, the Outreach Worker will serve as a mentor and peer counselor to a caseload of 10-15 individuals most impacted by community violence, redirecting them toward positive attitude, behavior, and lifestyle changes. JOB SUMMARY: As a key member of the SkillSpring (f/k/a GCD) department, the Workforce Programs Case Coordinator will engage with current participants in the young adult and high school programs in an effort to prepare for and secure employment. The role of the Workforce Programs Care Coordinator is to collaborate with SkillSpring participants using the care management process including: assessing a person’s functional level, their physical, cognitive, social-emotional status and their support system; developing a plan of care that addresses the needs and problems identified and incorporating the services that are needed to enhance the current support system; identifying and arranging for coordinated delivery of those services; monitoring changes in the person’s condition, and reassessing the person’s needs on a regular basis. This team member will serve in a coaching role and effectively advocate for program participants to have their various needs met with compassion and grace.
