Job Description The salary range for this position is $58,000-$65,000 annually RESPONSIBILITIES • Ensure health, safety & welfare of individuals., • Maintain contact with Volunteer Training Sites., • Ensure that agency vehicles are maintained., • Coordinate/Supervise transportation to and from program, volunteer training sites and recreation outings., • Report, document, and investigate incidents., • Act as a liaison with clinics providing Speech and Occupational services., • Maintain correspondence with parents and other providers., • Supervise distribution of petty cash., • Order supplies for program., • Ensure that nutritious snacks are purchased and provided for program individuals., • Assist in developing programmatic activities., • Interview/hire new employees., • Supervise and train Habilitation Specialists and ensure attendance of staff at annual core trainings., • Provide onsite training to Habilitation Specialists in areas of individual care and record keeping., • Review and complete billing documents., • Approve staff's time records., • Ensure that the Day Habilitation program services are adequate and/or appropriate to meet the needs of program participants and is compliant with all applicable federal, state and local laws, regulations and policies., • Maintain individual/family confidentiality., • Commitment to company values and adherence to policies., • Attendance and punctuality is essential., • Bachelor's Degree and/or substantial related supervisory experience., • NYS valid driver's license., • Extensive knowledge of OPWDD Regulations., • Ability to run., • Ability to safely assist lifting individuals of various weights & 20 lb items., • Must be able to communicate effectively with others and individuals served. Job Description RiseWell Community Services is seeking a full-time Case Manager to join our Residential Housing Team in Queens, NY. In this role, you will provide case management services, housing support, and assistance to residents with overcoming barriers associated with their health conditions so they may live successfully in on-site apartments. Provide information, training, hands-on assistance, and skill-building to individuals served. RiseWell’s Residential Services provide those in recovery from serious mental illness a safe, reliable home with on-site support. Our Residential team assists participants with an array of services aimed at supporting their recovery and improving their quality of life. Services include medication monitoring, skill-building development, symptom management, socialization, transportation training, connections to psychiatric and medical services, and more. Qualifications: BA in Human Services or equivalent. Experience working with people with disabilities or older adults a plus. Clean, valid NY State Driver’s License.
