Job Description POSITION SUMMARY: The Physician Assistant (PA) is a licensed clinical provider who practices in collaboration with physicians and the interdisciplinary care team at JPAFHC. In this role, the PA is responsible for assessing, diagnosing, and managing patients across our clinical sites, developing individualized care plans, and ensuring continuity of care and patient safety. The PA plays a critical role in supporting JPAFHC’s mission to deliver high-quality, patient-centered care, addressing both acute and chronic health needs, and expanding access to services for the underserved communities we serve. Essential Duties & Responsibilities Responsibilities listed are not all inclusive and may change over time to meet operational needs. Clinical Care • Provide comprehensive primary care services to patients across the lifespan, within the scope of practice of a licensed Physician Assistant and under the supervision of a Board Certified / eligible Physician in accordance with New York State regulations., • Diagnose and manage common acute and chronic conditions in collaboration with the supervising physician., • Perform medical histories, physical examinations, and clinical assessments to support accurate diagnosis and treatment., • Order, perform, and interpret diagnostic tests, laboratory work, and imaging studies as delegated and within scope., • Develop and implement patient centered treatment plans in collaboration with the supervising physician and care team., • Coordinate referrals to specialists and ensure appropriate follow up care., • Participate in rotational on call coverage to support continuity of care. Care Coordination & Team Collaboration • Work closely with nurses, medical assistants, behavioral health providers, social workers, and case managers., • Participate in team meetings, case reviews, and quality improvement initiatives., • Collaborate with schools, community organizations, and specialty providers to support patient care., • Ensure smooth transitions of care for patients with complex needs., • All providers will share responsibility for on-call coverage, ensuring patient care needs are met outside regular clinic hours. Documentation & Compliance • Document all patient encounters in the electronic health record (EHR) accurately, completely, and timely., • Lock notes within 48 hours of patient visit, • Comply with federal, state, and local regulations, including HRSA, UDS, OSHA, and infection control standards., • Support FQHC-specific requirements, including sliding fee scale documentation, enabling services referrals, and quality reporting. Quality Improvement & Population Health • Participate in quality improvement initiatives such as PCMH recognition, HEDIS/UDS measures, and chronic disease management programs., • Implement evidence-based guidelines to improve pediatric health outcomes., • Required to take part in peer review activities to ensure adherence to clinical standards and best practices. Community & Mission Engagement • Support the JPAFHC mission to provide accessible, equitable, and culturally competent care., • Participate in community outreach, health education events, and collaborations with local organizations., • Advocate for the communitiy’s access to preventive and primary care services. Work Environment & Physical Requirements • Outpatient clinic setting, with potential coverage at satellite sites., • Ability to sit, stand, and walk for extended periods; occasionally lift up to 25 lbs., • May require occasional evening or weekend hours depending on the health center’s needs. Preferred • Experience in FQHC, community health, or primary care settings., • Familiarity with PCMH, value-based care, or population health initiatives., • Bilingual skills (e.g., Spanish) highly desirable., • Experience with eClinicalWorks EHR systems
