The Manager, FP&A is responsible for supporting departments within the Enterprise, focusing on financial operations of the Technology and Telecommunications businesses. Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. The Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. They hold primary responsibility for developing program goals and methodologies and supporting DSPs to correctly implement and document such activities. The Client Coordinator ensures integrated services by fostering team awareness of service integration, coordinating service delivery with day program services, monitoring service provision by agency clinical staff, consultants, and community vendors, and ensuring services align with IDT treatment recommendations. They document progress and update Plans of Care / ISPs according to agency procedures. Responsibilities include the direct supervision of the Wellness and Reflections Unit; motivation, supervision and in-service training/education of the Resident Service Aides. The Director of Wellness will assure compliance with company policy and the State Health Department’s regulatory requirements for the admission and discharge procedure as well as appropriate documentation requirements. Additional responsibilities include: in-service education on health/wellness related topics and on the spot training to address identified performance concerns and/or improvements. Conduct as per company policy and State Department of Health requirements functional evaluations to identify Residents status and/or change in status and to determine the type and level of assistance needed: i. Develop and update each Resident’s service plan(s), evaluation, and other pertinent evaluations at specified intervals and when a change in Residents’ status warrants. • NJ only - If not licensed as an RN forwards all updated evaluations to the designated RN for sign off. ii. If delegated by ED, communicates change in Resident status to the Resident’s Representative and appropriate Administrative/Corporate Business staff; iii. Collaborate with a multidisciplinary team of providers, as well as the Resident and/or Resident’s Representative in the development of the Resident’s Individualized Service Plan on admission, appropriate revision within 30 days thereafter, and every 6 months; iv. Monitor the Wellness and Reflections Communication Logs daily for any unusual occurrences or changes in resident’s status and records and insures interventional strategies appropriately; v. Ensure all corporate policies and procedures and State Department of Health Regulations for Assisted Living Residences, Special Needs Program are followed and maintained; vi. Supervise Wellness and Reflections Unit staff to ensure that Residents receive appropriate care and activity program. vii. Ensure monthly support groups are conducted for families in the Reflections programs e. Coordinate care with the Department Heads, Resident’s physician, and other third-party providers: i. Collaborate with the Director of Case Management to assist residents with referrals to pertinent agencies when needed; i.e., Long Term Care insurance forms, nursing homes, social services agencies, Able Ride, etc. ii. Assist with hospital, skilled nursing facility, rehab centers, et al, with transfers and discharges and liaises external staff and discharge planner to determine suitability of Resident to return to facility: • Conduct off site visits as indicated/requested by ED/ Director of Case Management iii. Act as a liaison with Resident’s personal physician and other healthcare providers. iv. Refer Resident service problems to the appropriate Department Managers and Director of Case Management and Administration as necessary. f. Develop and keep accurate Resident records in compliance with company policy and regulatory rule: i. Collaborate in the development of the Resident service/care plans with Dietary Manager, (nutrition consult where indicated), Lifestyle Director, Director of Case Management, Resident’s primary care physician (and consultant specialists), third party providers (particularly home health) and the Resident’s family/Legal Representative; ii. Enter Wellness and Reflections Notes to include necessary social, recreational, physical, emotional, mental / behavioral health findings, needed and ordered interventions, and eventual outcomes iii. Review visiting and outside physician and provider notes and implement any changes in prescribed management as ordered by physician, g. Assure annual physician evaluations and where indicated behavioral health, pain, cognition, wander, fall, skin integrity evaluations; h. Oversee and document review of Residents who are self-administering medications to ensure continued competency and report all findings to ED, Director of Case Management, Resident’s physicians and Representatives – follow up as needed and indicated; i. Conduct skin observations on Residents, documenting findings to maintain compliance, and report findings for follow up. 3. Staff Education, Development, Supervision and Evaluation: a. Job Description JOB DEFINITION: The Facilities Associate will perform basic maintenance repairs to plumbing, electrical and support services. Although he/she may occasionally have use of the Agency vehicles, he/she will mostly be required to use his/her personal vehicle and personal maintenance tools. The Facilities Associate routinely interacts with all Agency staff and clients. EDUCATIONAL REQUIREMENTS: High school diploma or its equivalent EXPERIENCE REQUIRED: 1-2 years of relevant building maintenance experience. DUTIES AND RESPONSIBILITIES: • Perform routine maintenance and repairs to program sites and resident's homes, to include but not limited to: painting, masonry, woodwork, plumbing and electrical. Work will be delegated from work orders submitted by program directors and/or administrative personnel under the direction and oversight of the Facilities Supervisor., • Performs office relocations and/or resident relocations, as required;, • Assists with the relocation process including: moving furniture, removing debris, painting, repairing site for new residents., • Troubleshoots residential sites for potential maintenance and/or safety issues and makes recommendations to management regarding same. Suggests changes in working conditions and use of equipment to increase efficiency;, • Assists in acquiring vendor bids in collaboration with the Facilities Supervisor, Accounting Department and Director of Operations, • Follows safety regulations, company policies and enforces same when job assignment requires the assistance of temporary workers;, • Stands in for Facilities Supervisor as needed., • Identify and implement quality improvement trainings and activities as part of professional development goals, • Complete QI Training during onboarding and participate in QI refresher training as needed., • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
