Job Description About the Role The Direct Support Professional (DSP) provides person centered support based on the individual’s needs, preferences and choices. DSPs perform a wide variety of tasks and activities based on the valued outcomes of each person they work with and reflect the specific skills and abilities of each individual. DSPs encourage, guide and offer training to develop daily living skills, ensuring personal needs are met, promoting active engagement in community life and ensuring health and safety. DSPs create a positive and enriching program environment for all people supported. Direct Support Professionals are a vital member of each person’s support team. DSPs are expected to align their performance with the Code of Ethics and OPWDD DSP Competencies. What You’ll Do • Supports the person’s developmental needs, desires, preferences, and choices., • Provides support with the person’s goals as outlined by their Staff Action and Life Plan., • Escort people supported the community for both recreational purposes and with healthcare professionals as needed., • Operate agency vehicle to support programmatic needs, • Assist with meal planning and preparation, • Provide the necessary level of supervision and support for a person’s needs (behavioral, medical developmental, emotional etc.), • Safely administer medication under the license of the RN as described in Approved Medication Administration Personnel (AMAP) training (if applicable), • Be knowledgeable of all OPWDD, NYSJC, and organizational policies and adhere to them accordingly, • Ensures that the program environment is clean, safe and offers a positive-home like environment, • Complete all mandated OPWDD and Rising Ground Trainings, • Act as a mandated reporter in accordance with OPWDD, NYSJC, and Organizational Policies, • Notify program RN or appropriate administrator when a concern arises that may impact on the health or safety of the people supported., • Contemporaneously complete all documentation including, but not limited to monthly progress notes, daily billing, medical follow-up, clinical data or other communications as assigned, • Other Duties assigned based on programmatic need Your Qualifications • High School Diploma/GED Required, • Must be able to work independently and self-directed., • Must be able to use technology (for completion of billing data and other assigned tasks), • Strong written and verbal communication skills., • Positive attitude toward the challenges faced by the people that we support., • Must be flexible with meeting the needs of the people they support., • Availability to meet the schedule of the position as assigned, with occasional flexibility, • Prior experience with OPWDD or people with disabilities is a plus Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities required of the employee for this job. Job Description Summary: The Senior Case Manager provides direct assistance, either in-person or telephonically, to victims of crime and abuse, specializing in working with survivors of intimate partner and family violence (includes sexual assault and families of homicide victims). The Senior Case Manager will provide services to victims of crime and abuse. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Conducts client centered safety assessment and risk management planning;, • Provides crisis intervention and direct assistance to victims of crime and abuse; develop creative solutions reflecting clients’ unique needs in collaboration with client, • Advocates for clients for public entitlements with systems and other agencies, where necessary, such as obtaining orders of protection, help with hospital/medical related needs; Office of Victims Services compensation, and relocation assistance;, • Provides information and referrals and facilitates linkages for clients for services such as shelters, mental health treatment, public assistance and other entitlements based on assessment and clients’ needs;, • Assesses for and provides emergency practical assistance for clients including transportation, food vouchers, financial assistance and lock replacement in accordance with agency policy;, • Engages in regular supervision meetings with supervisors to review cases, reflect on client interaction and build skills;, • Maintains thorough and accurate case records and statistics in web-based case management system;, • Collects client data, tests new interventions and tracks outcome;, • Participates in staff meetings, individual and group supervision, community meetings and relevant trainings and seminars;, • Conducts educational presentations and trainings and participates in outreach events for community groups about intimate partner and family violence, victim assistance resources and other related topics;, • Works collaboratively with other Safe Horizon programs to facilitate linkages for clients;, • Safe Horizon staff, • Criminal justice personnel, • District Attorney’s Office, • NYPD, • Office of Victim Services, • ACS, • NYCHA and Section 8 staff Required: • Must be sensitive towards and understanding of victimization;, • Must be able to demonstrate respect, compassion and a non-judgmental approach to victims of crime and abuse;, • Must be willing to participate in a learning community;, • Must be well organized and detail oriented;, • Must demonstrate the ability to work independently and be resourceful;, • Must demonstrate leadership skills (acts as a “go-to” person for colleagues, takes initiative; suggests ways to make processes work better);, • Must be a team player;, • Possess strong communication and organizational skills;, • At least three years of prior direct service experience in a social service setting is required – Or- a minimum of two years of case management experience at Safe Horizon is preferred.
