Job Description Astor’s Prevention Services/Family Connections is a time-limited, evidence-based model that utilizes a home-based approach to address both the concrete and clinical needs of the family. Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family. The Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 12-14 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. HMH is hiring for a full-time Behavioral Health Clinician responsible for direct clinical work with clients in the outpatient mental health clinic, with a priority focus on those in the CONNECT program. The BH Clinician will also be responsible for coordinating and accessing care for their clients. The CONNECT program is a Department of Health and Mental Hygiene (DOHMH) Demonstration Project to provide rapid access to mental health services in a more flexible and holistic way with the community and the whole person outside the clinic walls and beyond the traditional doctor/therapist-patient interaction. Job Description JOB SUMMARY The Family Specialist/Homemaker and Support Partner will provide needed care coordination, outreach, support, advocacy, information and referral to New Ground families.
