Why You'll Enjoy This Opportunity • Serve an underserved geriatric population where your clinical expertise has a meaningful daily impact, • Practice exclusively within skilled nursing and rehabilitation facilities throughout the Queens area, • Strong compensation package with excellent long-term earning potential, • Flexible scheduling designed to support work-life balance, • Practice independently while benefiting from experienced clinical leadership and operational support, • Build lasting relationships with residents, families, and facility care teams, • Develop advanced expertise in geriatric psychiatry and behavioral medicine, • Join a growing behavioral health organization committed to provider development and internal advancement, • Opportunities to expand into leadership, mentoring, multi-site coverage, and program development as the organization grows, • Structured clinical model allowing providers to focus on patient care rather than administrative burden Requirements • Perform comprehensive psychiatric assessments, diagnostic evaluations, and routine follow-up visits for residents within skilled nursing and long-term care facilities, • Evaluate, diagnose, and manage psychiatric disorders affecting adult and geriatric patients, including depression, anxiety, bipolar disorder, psychotic disorders, dementia-related behavioral symptoms, adjustment disorders, chronic mental illness, sleep disturbances, and behavioral complications associated with neurocognitive disorders, • Provide comprehensive psychopharmacology management, including medication initiation, dosage adjustments, monitoring for effectiveness, and management of side effects, • Assess residents experiencing behavioral changes, cognitive decline, agitation, aggression, hallucinations, mood instability, or other neuropsychiatric symptoms, • Develop individualized treatment plans that incorporate psychiatric, medical, cognitive, functional, and psychosocial considerations, • Partner closely with attending physicians, nursing staff, rehabilitation therapists, social workers, case managers, and medical directors to coordinate comprehensive patient care, • Participate in interdisciplinary care conferences and provide behavioral health recommendations to improve resident outcomes, • Educate residents, family members, caregivers, and facility staff regarding psychiatric diagnoses, medication management, behavioral interventions, and treatment expectations, • Promote continuity of psychiatric care while helping reduce unnecessary hospital transfers and behavioral crises, • Maintain timely, accurate, and compliant documentation in accordance with facility policies and regulatory requirements, • Contribute to quality improvement initiatives designed to enhance behavioral healthcare services within long-term care settings Qualifications • Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC), • Active and unrestricted New York State Nurse Practitioner license, • DEA registration or ability to obtain, • Strong clinical knowledge of psychiatric assessment, diagnosis, psychopharmacology, and evidence-based treatment planning, • Ability to independently manage a psychiatric caseload while working collaboratively within interdisciplinary healthcare teams, • Previous experience in psychiatry, behavioral health, skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric medicine preferred, • Experience treating dementia-related behaviors, chronic psychiatric illness, mood disorders, anxiety disorders, psychosis, and medically complex patients is highly desirable, • Excellent communication, organization, and relationship-building skills, • Passion for caring for aging adults and improving access to behavioral healthcare for vulnerable populations Career Development This opportunity offers significantly more than a traditional PMHNP position.
