Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. The Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. They hold primary responsibility for developing program goals and methodologies and supporting DSPs to correctly implement and document such activities. The Client Coordinator ensures integrated services by fostering team awareness of service integration, coordinating service delivery with day program services, monitoring service provision by agency clinical staff, consultants, and community vendors, and ensuring services align with IDT treatment recommendations. They ensure a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to maintain adequate coverage levels and ensure an atmosphere conducive to development and well-being. They document progress and update Plans of Care / ISPs according to agency procedures. The Client Coordinator reports regularly to the Supervisor regarding program compliance, noncompliance, and remedial actions. They identify needs and request support as necessary. They participate in clinical reviews, incident reviews, human rights committee meetings, and other meetings as directed. As schedule allows, the Client Coordinator serves as primary chair for Life Plan meetings. They review documentation and provide technical assistance to program management and staff. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES The employee must be able to interact with individuals and staff professionally and ethically, understand and implement service plans, and complete required documentation clearly, legibly, and on time. Previous experience working with individuals with IDD/mental illness is required, as well as experience managing teams. Competency in Microsoft Word is required. Experience working in a Day Support Program or Special Education environment is preferred. The employee must have: • A passion for working with individuals with intellectual and developmental disabilities., • Knowledge of basic human physical and psychological needs., • Knowledge of health and safety precautions, first aid, behavior management, and hygiene., • Ability to write progress and behavior reports professionally., • Ability to use computers and electronic equipment., • Ability to read and comprehend training manuals., • Ability to follow instructions, learn adaptive equipment, be supportive, patient, and a team player., • Ability to observe, record, and report participant behavior and physical condition., • Ability to prioritize conflicting deadlines. (All numbering below is kept exactly as written but corrected for grammar.) • Maintain all individual Day Plans and update each plan as needed., • Attend all Individualized Service Plan meetings and applicable reviews., • Develop goals for individuals, review progress daily, track data monthly, document outcomes, and make recommendations., • Assist individuals and advocates in obtaining services outlined in the Day Habilitation Plan., • Complete monthly progress notes for all participating individuals., • Maintain and update all individual Habilitation Plans., • Identify required trainings for DSPs., • Handle all issues involving parents of assigned individuals., • Communicate family and advocate concerns with management., • Conduct frequent program observations to ensure proper implementation of goals., • Conduct compliance reviews, monitor events and incidents, identify risks, develop corrective plans, and review documentation to ensure compliance., • Maintain program records and ensure compliance with Life Plans, Staff Actions, IPOP, Assessments, LOC, and DDP2, ensuring consistency with clinical plans such as PONS and BSP., • Evaluate individual strengths and needs and develop Habilitation Plans accordingly., • Participate in Life Plan development related to day program services., • Develop and supervise SAP/DHP goals and the IPOP., • Monitor progress and complete monthly progress notes., • Review all IDT summaries and ensure follow-up., • Ensure timely completion of monthly, semi-annual, and annual summaries; submit reviews to Care Managers., • Attend Life Plan meetings, monthly staff meetings, and IDT meetings., • Maintain effective communication with teams, families, and staff., • Complete addendums or updates to Habilitation Plans as needed., • Maintain contact with families, Care Managers, clinicians, residences, and other programs., • Review and update IPOP, Assessments, and other plans annually or as needed., • Participate in internal audits and follow up on deficiencies., • Ensure Life Plans, Staff Actions, Monthly Notes, and service documentation meet OPWDD and OMIG requirements., • Review weekly billing and approve documentation for billing., • Notify team members and families of all meetings and habilitation changes., • Train staff on Habilitation Plans., • Assist Program Coordinator in leading team meetings., • Assist Program Coordinator with admissions and discharges., • Coordinate the development and review of individual plans of care and supervise rehabilitative services., • Document monthly or semi-annual progress and outcomes., • Remain familiar with individual programming plans and status., • Maintain uniform record-keeping consistent with confidentiality and agency standards., • Enforce company policies and procedures., • Support the organization’s mission and vision. Required: • Associate Degree in a Human Services field with 4–6 years of OPWDD experience, OR, • High School Diploma/GED with 7–10 years of OPWDD experience., • Experience handling confidential information, multitasking, problem-solving, taking initiative, working independently, and meeting deadlines., • Successful completion of required trainings (SCIP, AMAP, CPR/First Aid).
