Job Summary The Habilitation Specialist (DSP), is responsible for assisting the people we support, while following established treatment plans. QSAC’s Residential program emphasizes the importance of promoting independence through the development of daily living/social skills and integration into the community through recreational outings. The Case Coordinator will support participants in adhering to supervision schedules and legal proceedings, conduct regular assessments to identify service needs, and create collaborative service plans. The Case Coordinator emphasizes providing strength-based, gender-specific, and culturally sensitive services, guided by the principles of risk, need, and responsivity. The aim is to reduce recidivism, foster community treatment engagement, and assist participants in achieving their personal goals for recovery and integration. The Case Coordinator should excel in teamwork, collaborate closely with a Peer Specialist, and be prepared for court duties and community fieldwork within NYC.
