Salary: $85k-100k What We Offer! • Generous paid vacation, sick, personal time, and mental health days., • A rich benefits package, including medical, dental, life and long-term disability insurance, • 401(k) plan with company matching options, • Tuition Reimbursement Program, • New York’s 529 College Savings Program, • Leadership and Master Clinician Educational Tracks, • Progressive Clinic Director and Leadership Development Programs, • Continuing Education & Professional Development, • (1) year Paid OCS Residency Program, • Up to a $1,500 stipend per year, plus all expenses paid for any specialty certifications towards Schroth, Pelvic Floor, OCS, PCS, NGS or WCS, • Unlimited online CEU’s through our partnership with MedBridge Education, • Focus on Patient Care - Quick EMR (No Oasis required), • We offer Flexibility and Autonomy - see patients any 7 days of the week, • Preference given to see patients in your primary geographic areaRequirements:, • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance., • Accept patients within your geographical area of coverage, • Plan, prepare and carry out individually designed programs which provide skilled therapy to patients to restore overall function and independence in the home setting., • Develop a plan of care/goals in consultation with the physician, family and patient., • Timely and accurate documentation of visit, • Physical Therapist Program Graduate., • NY Physical Therapist License or eligible., • Evaluates a patient's need for physical therapy and develops an appropriate treatment plan., • Provide appropriate care and treatment to patients based on physician care plan, agency policy and professional standards of care., • Instructs family members, PTA's, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
