The Case Manager will be responsible for conducting client assessments, developing Independent Living Plans, linking clients with community-based service. QUALIFICATIONS Bachelor’s degree in Human Services required. Minimum of 1 year of experience in human services or related field preferred. Knowledge of the Suffolk County Department of Social Services preferred. Knowledge of human services and previous experience working with at-risk population preferred. Excellent interpersonal, verbal, and written communication skills required. Proficient computer skills, including Microsoft Office required; experience with HMIS, Google Apps, and web-based project management software preferred. Bilingual in Spanish preferred.
