Additionally, SOS provides support during the period of transition from street homelessness to stable housing. SOS Teams are comprised of licensed behavioral health clinicians, registered nurses, care managers, and peer specialists. This is an excellent opportunity to play a pivotal role in the Governor’s newly launched, innovative Safe Options Support (SOS) program, which will provide comprehensive care to street homeless or subway dwelling individuals. Our full time Administrative Assistant is responsible for office operations including managing the reception desk, answering phones, greeting clients, data entry and records filing, receiving and sending mail, and supplies/equipment inventory and purchasing. Job Description MST Therapist About the Position: You will receive ongoing training in the Multisystemic Therapy (MST) model as you work with families, youth, their communities, and other key members of their ecology to implement MST as designed. Extensive research has proven the effectiveness of MST. MST therapists do whatever it takes, via close collaboration with all involved, to address the needs of adolescents returning from residential placement for mental health, trauma and, in some cases, substance abuse. Included in this is accompanying the client to their assigned unit and ensuring the unit is fit for a new admission. The Shift Supervisor ensures all clients under the age of two are provided a crib upon their entry into the shelter and all new intakes are provided with linen, toiletries, kitchen utensils, and any/all other supplies issued to new clients.The Shift Supervisor provides orientation to all new admissions.The Shift Supervisor oversees the distribution of food from our onsite food pantry.The Shift Supervisor conducts periodic facility evacuations, periodic fire drills, and routine site tours(Walk through) and maintains the required records of all such drills and tours. The Shift Supervisor supervises surveillance of the grounds, facility, and any activities of residents to prevent theft, unauthorized entrances, fire, and accidents. The Shift Supervisor must maintain a visible presence throughout the facility during the entirety of his or her shift.The shift supervisor will conduct a monthly inspection of all fire extinguishers in the building an maintain log recording the monthly check, shift supervisor will also execute a work order for any fire extinguisher deficiencies found. The Shift Supervisor is responsible for ensuring the agency vehicle is properly signed out and all logging information is dually completed.The Shift Supervisor records and reports maintenance needs of the building (i.e. work order request).The Shift Supervisor attends all required staff meetings and scheduled trainings even when scheduled on a shift that is not regularly scheduled for them.The Shift Supervisor mediates client disputes and maintains logs of unusual events or activities.The Shift Supervisor interfaces with relevant public service/community entities, i.e.: police, fire department and hospitals in efforts to obtain emergency services when needed and assumes responsibility for obtaining emergency services when occasions arrive.Work hours are to be arranged with administration to accommodate the needs of the facility. Employment hours may be altered (with as little as two-week notice) depending on the administrator’s and/or facility’s needs. Funded by the New York City Department of Health and Mental Hygiene (NYC DOHMH), the Harlem Health Advocacy Partners (HHAP) program aims to improve the health of NYC residents by supporting access to health and social services to which they are entitled, providing health coaching to individuals to increase self-efficacy for healthy behavioral changes and disease management, building community capacity to seek and/or create healthy conditions and acceptable services through advocacy to government and other service providers, and providing Group Wellness Activities and high-quality educational workshops. The CHW-HHA works independently with clients to do outreach, engagement, case management, and follow up, as well as closely with the HHAP team and supervisor to share experiences, identify unresolved issues, share best practices, and participate in problem-solving. They will work under the supervision of a CHW-HHA Supervisor, and work closely with other members of the HHAP team, with interaction with other teams within PHS’ Neighborhood Health Division as needed. Specifically, the Community Health Worker will: Health coaching activities: • Provide in-person or remote health coaching services, maintaining an annual caseload as determined by supervisor (maximum 30 health coaching enrollees at one time)., • Conduct established educational curriculum for Health Coaching, including:, • Delivery of peer health/education coaching support to chronically ill participants and residents in need of assistance with social determinants of health, following the Health Coaching protocol developed and provided by DOHMH.
