Ø Responsible for completing a face-to-face initial/ongoing Comprehensive Assessment (CA) with consumers.
Ø Interview consumers to obtain information concerning their medical history and other pertinent information.
Ø Assess the counseling needs of the consumers.
Ø Assess consumers using the ASI or other standardized clinical interview tool and diagnose consumers for substance use disorder using the DSM-5.
Ø Determine the appropriate level of care according to ASAM Criteria
Ø Obtain previous records that are relevant to the current treatment episode unless clinically contraindicated, and collaborate with the consumer to develop a written treatment plan that includes goals and measurable objectives, and is consumer-centered, and recovery-oriented.
Ø Provide the substance abuse counseling services specified in the consumer treatment plan.
Ø Review consumers throughout the treatment episode according to the ASAM Criteria, to determine the need for continued services, transfer, or discharge/transfer.
Ø Review and, where necessary, revise the consumer’s treatment plan to address ongoing problems.
Ø Develop a consumer discharge/transfer plan to ensure movement to the appropriate levels of care.
Ø Participate as a member of a multidisciplinary team for assigned consumers.
Ø Provide active case consultation.
Ø Document all counseling and education services, assessments, reassessments, referrals, and follow-ups in the consumer's clinical record, and provide appropriate signatures and dating of such entries, including those made in electronic records.
Ø Retains the responsibility of meeting face-to-face with the Qualified Professional (QP) Care Managers Staff to review the CA and Treatment plan of the consumer for the development of an action plan to meet the consumer’s needs.
Ø Progress notes due daily.
Ø Report directly to the Chief Executive Officer (CEO).
