Job Description POSITION TITLE: Triple P Bilingual Care Manager REPORTS TO: Triple P Manager BROAD FUNCTION: The Triple P Bilingual Care Manager will work alongside the Practitioners, throughout service delivery. The Care Manager will connect with families directly and act as a resource broker providing resources and linking families services. I. Key Responsibilities: Family Care Coordination • Provide concrete support to families that are receiving Triple P services., • Identify gaps and barriers to services, advocating for families as needed., • Provide on-going assessment of families by engaging and partnering with parents throughout the course of program participation., • Refer families to services in a timely manner, prioritizing those based on severity of needs, • Ensure the successful linkage to community resources by providing transportation, if needed.Teaming, • Collaborate with DCP&P and other referral partners to connect families with community resources to meet family needs., • Develop and maintain relationships with a diverse array of community programs to effectively meet the needs of individual families., • Collaborate with Practitioners and Supervisor to identify immediate service needs or resources for families., • Participate in advisory councils/boards within their local community/area of service to gather beneficial information to assist in supporting families.Development of Professional Self:, • Participate in agency activities, required meetings, and trainings as required by agency and/or DCF., • Complete the Positive Parenting Program training and other trainings required by Family Connections and/or DCF., • Regularly research and document community resources including services related to health, housing, education, employment, and social supports., • Share any updated resource information with the Practitioner and Supervisor.Monitoring and Reporting:, • Complete required documentation, forms, and statistical reports., • Participate in quality improvement and evaluation activities to enhance service delivery.Minimum Qualifications, • Education: Bachelor’s degree in Psychology, Social Work, or equivalent experience within the child welfare system preferred., • Minimum of at least one years’ experience in parent/family support, maternal-child health., • Candidates should also demonstrate knowledge and experience in care coordination for families and community stakeholders., • Proficiency in Spanish Skills and Abilities:, • Experienced in parent/family support, maternal-child health, or adolescent mental health., • Demonstrates knowledge and experience in care coordination for families and community stakeholders., • Embedded in the communities that are being served., • Demonstrates compassion and empathy in relating to clients with diverse needs., • Cultural competency and ability to work with diverse populations., • Translates complex concepts into easily understandable language for families., • Shows awareness of cultural diversity and its impact on service planning and provision., • Valid driver’s license and reliable transportation., • Has the ability to work on a multi-disciplinary team., • Has the ability to use a computer database system (EHR)., • Bilingual staff must demonstrate the ability to communicate clearly, accurately, and professionally in both English and the identified second language across various contexts, including verbal interactions, written documentation, and interpretation or translation tasks as applicable to their role., • The use of translation aids or tools, such as Google Translate or similar applications, is not permitted as a substitute for language proficiency in fulfilling bilingual job duties.Salary: $55,000 Vacation & Benefits: • Vacation: As a full-time employee working generally 35 hours per week, you will be eligible to participate in the Family Connections group benefit plans. Benefits include medical, dental, life insurance, long term disability, retirement plans, and additional supplementary benefits. Yes, you can elect to not enroll into these benefits. You will also be eligible to accrue vacation, personal, and sick days in addition to 12 paid holidays, and summer wellness Fridays. Work Location: • This position involves working in clients’ homes and community settings.Schedule:, • Evening and weekend work may be required to meet the needs of families.
