Notify all participants including outside collaterals of the meeting in a timely fashion., • (cooking, arts & crafts, trips, etc.) and familiarize other staff with any changes to ongoing individuals’ plan, • Accompany individuals to community activities (including volunteer experiences) and on trips. Carry out pertinent methodologies with individuals while in the community., • Maintain and thoroughly clean equipment and supplies, • Carry out assignments and/or activities as directed by the professional/supervisory staff., • Assist with transporting consumers to/from the Day Habilitation Program, to/from their community placements, and to/from other activities as directed., • Act as the liaison to parents, residences, MSC’s, etc. Ensure that they receive pertinent information. POSITION SUMMARY: The Housing Specialist is responsible for working with birth parents and youth in our Family Foster Care and Residential programs.
