Job Summary: The NYC Family Justice Centers (FJCs) are an initiative of the Mayor’s Office to End Domestic and Gender-Based Violence to reduce barriers faced by survivors of domestic and gender-based violence (GBV). GBV can include intimate partner and family violence, elder abuse, sexual assault, stalking, and human trafficking. The NYC Family Justice Centers are co‐located multidisciplinary service centers providing vital social services, civil legal, and criminal justice assistance for survivors of domestic and gender-based violence and their children—all under one roof. Arab American Family Support Center is co-located at the FJCs and partners with the Mayor’s Office, the District Attorney’s Office, and other civil legal and community-based organizations to provide a safe and welcoming place for victims to receive various services. AAFSCNY staff at the FJC provides crisis intervention, case management services, advocacy, information and referrals, and practical assistance to survivors, providing client-centered safety assessments and risk management plans. Client Navigators will greet clients in the waiting areas and hospitality suites and escort them to a private meeting space where they will explain the FJC service delivery model and conduct a brief initial service need and safety assessment with the goal of connecting the client to the appropriate services at the FJC’s. They will continue to help clients navigate the FJC service model and connect successfully to services and partner agencies. Essential Duties: * Welcome clients to the FJC and provide a detailed overview of the FJC service delivery model including confidentiality policies, on-site referral process and available services. * Screen clients to assess eligibility for FJC services and their service needs, including cultural, linguistic, legal, practical and safety-related needs. * Call back voicemails left on the FJC’s main phone line and do virtual screenings as needed. * Notify AAFSC Supervisor and ENDGBV/FJC administrative staff of high-risk clients as appropriate. * Address immediate client needs and provide crisis intervention. If no case manager is available to assign the client to, the Client Navigator will provide safety planning and advocacy as needed. * Work with clients to determine appropriate referrals for case management, mental health counseling, civil legal, economic empowerment, and law enforcement personnel; complete appropriate forms, including the FJC Referral and Consent Forms; and communicate any pertinent information to the partner staff member to help best meet clients’ needs and concerns. * For established FJC clients with an already assigned case manager who come to the FJC as a walk-in, conduct a brief screening and connect to their assigned case manager. If the case manager is not available, conduct a safety assessment and safety plan as needed. * Provide service referrals and warm hand-offs/linkages, if it is determined that a client is not eligible for FJC services. * Maintain communication with clients waiting in the waiting areas and hospitality suite after the initial client screening to answer questions, provide information and provide updates about estimated wait times. * Speak with AAFSC supervisor and ENDGBV/FJC administrative staff if a client is distressed if a client asks to speak to a supervisor and/or if the client is experiencing a physical or mental health emergency. * Work collaboratively with the reception staff at the relevant District Attorney’s Office to refer appropriate clients and make appointments for those clients, as needed. * Enter client information into the FJC client application (aka database) on a daily basis, and track additional data or client needs as requested by ENDGBV. * Support ENDGBV’s ongoing efforts to solicit client feedback regarding FJC operations including notifying all clients of their opportunity to complete a client satisfaction survey after every FJC visit and contacting clients after their visits to the FJC’s to complete the surveys and reconnect them to any needed follow-up services as appropriate; * Provide coverage for the Front- Line Advocate, as needed and appropriate. * Adhere to AAFSC and FJC confidentiality practices and protocols. * Other tasks that enhance the mission of the FJCs, as mutually agreed upon by AAFSC and ENDGBV. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Nurse Practitioner will be matched with the population that most closely meets their clinical expertise, desired work location and schedule, and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs, • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization, and escalate care when clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care., • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner., • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication., • Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed., • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics., • Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist., • Provide clinical oversight for psychiatric nurse practitioner trainees, including appropriate direct supervision and documentation of evaluation and feedback., • Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director, or Chief of Service for Psychiatry, or their respective designees., • Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Summary of position The Senior Psychiatrist is the psychiatric leader of the facility or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Supervising Psychiatrists (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. Responsibilities Include (depending on location): • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals., • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review., • Close collaboration, including regularly scheduled meetings, with other relevant disciplines within the mental health, medical, nursing and operations departments, as well as the Department of Correction., • Ensure that medication renewals are completed in a timely manner., • Educate clinical staff about new developments regarding best practices in all aspects of psychiatric care, with specific attention to the challenges and issues facing a jail setting. (e.g., influence of legal case, dual loyalty, medication diversion), • Ensure compliance with all regulatory standards., • In collaboration with the Mental Health Unit Chief, oversee the orientation of new staff to the facility or program., • Conduct regular and multi-disciplinary case conferences., • Report on an as-needed basis to the CHS Morbidity and Mortality review committee., • Work in collaboration with the Mental Health Unit Chief to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education., • Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary., • Lead at least monthly interdisciplinary mental health staff meetings in coordination with the Mental Health Unit Chief and Clinical Supervisor., • Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints., • At least 30% of time devoted to direct clinical care, which may include direct supervision of psychiatric trainees. For smaller facilities or programs with less supervisory need, more direct patient care will be expected., • On-call responsibilities for two weeks at a time, one to times per year, for off-hours issues that require high level clinical intervention. Work Shifts Time: 8a-4p Days: Mon-Fri Facility: CDU-WF facility on Rikers Island Duties & Responsibilities The Quality Assurance (QA) Radiological Technologist position encompasses responsible, administrative and clinical activity of varying degrees of latitude. Clinical level judgement is needed in the daily supervision of radiological functions and patient care, and departmental projects. Responsibilities include: • Review exam orders, consult with radiologist, schedule patients and arrange for transportation as indicated., • Identify patient and document all information pertinent to final report in the RIS and on images (PACS)., • Prepare all patients (infants, children, adolescents, adults and geriatric) for radiological procedures requested by the clinician. Arrange for proper protection of patients, public and staff from infectious diseases/materials., • Perform all general diagnostic imaging and routine examination in CT and Angiography., • Assists in performing daily checks of radiographic imaging equipment, immobilization devices and other necessary accessories., • Enters data and maintains documents and logs as required and mandated by the department QA guidelines and various regulatory agencies., • Maintains adequate stocks of supplies and materials needed for use in the department., • Checks equipment for defects prior for use and assures proper use of such equipment through instruction and review., • Report any unexpected patient incidents in accordance with the occurrence report protocol., • Maintain and exercise all mandated radiation safety regulations. Maintain basic care of all the imaging equipment. All malfunctions are to be reported to management staff whether mechanical, electrical or suspect and service request documentation completed. Monitor PACS and RIS incomplete worklists to ensure all exams are completed., • In accordance with HIPAA (the Health Insurance Portability and Accountability Act), staff members will exercise due diligence in ensuring patient privacy and confidentiality., • Identifies activities that the network has implemented to promote patient safety and takes all necessary precautions to ensure a safe environment for patients, visitors and staff. In addition to the Radiology Technologist job functions, the QA Technologists is responsible for the following: • Organize and file maintenance reports for each building in regards to dentistry and radiology x-ray equipment and ensure dentistry and x-ray equipment follow the Department of Health codes, policies and procedures., • Participates in quality assurance activities, including coordination, control and maintenance of technical equipment., • Establish a daily/weekly/monthly quality control routine for monitoring, evaluating, and maintenance of X-ray equipment to ensure optimal performance and stability., • Documenting and maintaining records for the quality control program in accordance with applicable regulations, legal requirements, accrediting agencies and recommendations from equipment manufacturers., • Performs research and participates in special projects involving evaluation of equipment and delivery of service., • Work with our radiologists to obtain updated documentation of primary diagnostic monitors., • Distribute/collect dosimeter badges from each employee/facility that works with x-ray equipment., • Recommends area assignment of personnel to coordinate activities within the department with other activities and services and represents the department in interdepartmental operations and meetings., • May be assigned duties and responsibilities of X-ray Director in regards to radiology department workflow. Minimum Qualifications Assignment Level III 1. For Supervisory Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and two (2) years of Radiologic Technologist experience performing duties consistent with Assignment Levels I and II, one (1) year of which must have been in a supervisory capacity., 2. For Specialized Imaging Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and possession of a current advanced- level certification for the performance of these procedures.
