The Case Manager will be responsible for engaging new referrals of prospective participants to determine eligibility for Healthy Families. The Case Manager will also conduct interviews and complete assessments with expectant parents or parents who have a child less than three months of age. The Case Manager will provide home visits to families located throughout Nassau County, but primarily in Hempstead, Uniondale, and Roosevelt. Travel is required. Job Summary The Academy Charter School Art Teacher understands and promotes the school’s mission, vision, objectives, and policies both within and outside the immediate school community.
