QSAC’s Residential program emphasizes the importance of promoting independence through the development of daily living/social skills and integration into the community through recreational outings. Job Description Essential Job Functions: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients, • Attempt to reach by phone and mail victims who have filed reports with the precinct or PSA with a specific focus on victims of violent felony crimes and those victims who have filed reports two or more reports in the previous 12 months, • Work with victims of crime who come to the precinct or PSA seeking assistance, • Conduct home visits with officers and detectives to assist victims as needed, • Provide information, referrals, and practical assistance to victims experiencing crime, • Conduct client centered safety assessments and safety planning suited to the individual’s needs, • Provide crisis intervention and supportive listening, • Advocate with NYPD personnel that may include keeping a victim apprised of an NYPD investigation, • Provide criminal justice advocacy, • Advocate for victims for public benefits they may be entitled to, • Help with applying for compensation through the New York State Office of Victims Services (OVS), • Coordinate community resources for crime victims, • Provide information and referral for victims for services such as shelter, mental health treatment, medical care, and other entitlements, • Provide emergency practical assistance for victims including transportation and food vouchers, • Work collaboratively with police officers to effectively aid victims, • Collect data, track the history of crimes, and prepare monthly reports, • Perform data entry into a computerized record keeping system and maintain thorough accurate, and timely case records and statistics, • Advocate with Family and Criminal Court staff for victims, • Provide coverage in vacant precincts as needed, • Conduct trainings with police personnel, • Conduct community presentations, • Attend relevant trainings, workshops and seminars, • Work with other victims of domestic violence as needed, • All internal Safe Horizon programs and departments that can comprehensively support the client and case plan, • All external agencies and organizations that can provide appropriate services to the client, specifically the NYPD and the court systems, • The district attorney’s office in each of the boroughs, including the domestic violence bureaus, • Clear and concise verbal and written communication skills, • Excellent interpersonal skills, be assertive and well organized, • A minimum of two years’ experience in crime victim service delivery, or other relevant experience, • Basic knowledge of crime victim and domestic violence issues, • Basic understanding of computer and systems for managing case records, • Bilingual ability may be required to meet individual precinct/community linguistic needed This position is on-site. The project consists of 23 residential units, 5 singles and 18 families with CAMBA, Inc. providing the on-site services Position: Case Manager Reports To: Program Manager Location: 1047 Beach 21st Street, Far Rockaway, NY 11691 What The Case Manager Does: • Maintain professional relationships with clients and client confidentiality., • Practice Universal Precautions/Standard Protocol & Procedures., • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information., • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building)., • Act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc., • Schedule appointments for tenant with referral organizations., • Escort tenants to appointments (educational, medical, social service, etc.), • Assist tenants in completing applications for benefits and entitlements and may process applications on tenants' behalf., • Follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • Evaluate actual living conditions of tenants through home visits., • May prepare marketing materials for the program., • May prescreen tenants over the telephone for eligibility and may schedule intake appointments., • Input tenant data and tenant progress information into automated database Minimum Education/Experience Required: • Bachelor’s degree (e.g., B.A., B.S.W.), • Must be fingerprinted and obtain “Criminal History Check “(CHC) with Justice Center for the Protection of Vulnerable People with Special Needs Ability to maintain fingerprint clearance throughout the duration o employment., • As a Mandated Reporter, you must immediately report suspected incidents/cases abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline at 1-855-373-2122 and complete a CAMBA Incident Report, • Flexible hours may be needed occasionally to provide shift coverage until 8 pm.
