Intervene as advocate for clients in order to resolve emergency problems in crisis situations. Referrals to the Social Services agencies in community such as (shelters, welfare). Maintain client records and ensure documentation is in compliance with DMHAS and agency requirements and standards. Schedule and organize treatment team meetings and keep updated records and files of all programs Provide quality case management and maintain phone regular contact with the individual, natural supports, and social service agencies (when applicable) to provide updates on treatment and progress. Follow progress of discharged patients in order to determine effectiveness of treatments. Attendance and participation in supervision with clinical supervisor/director. Attendance and participation in case conferences and case consultation with interdisciplinary staff. Work with other agencies and promotes good community relations through involvement in community events and other marketing events. Implement specific measures for program growth. Attendance and participation in scheduled in-service trainings, meetings, and other trainings required by DMHAS and the agency.
